摘要
目的探究蜕膜组织中促血管生成素-1(Ang-1)、促血管生成素-1(Ang-2)及血管内皮生长因子(VEGF)的表达与体外受精胚胎移植(IVF-ET)术后稽留流产的关系,并分析IVF-ET术后稽留流产的影响因素,为预防IVF-ET术后流产提供依据。方法选取我院于2016年1月~2018年1月收治的82例IVF-ET术后稽留流产孕妇作为本次研究的观察组,选择同期82例自愿行人工流产孕妇作为本次研究的对照1组。两组孕妇流产后获取蜕膜组织并测定Ang-1、Ang-2及VEGF的表达,比较两组孕妇蜕膜组织中Ang-1、Ang-2及VEGF的表达。另外,选择同期82例IVF-ET术后继续妊娠者为本次研究的对照2组,比较两组孕妇临床一般资料,用Logistic回归分析其流产影响因素。结果观察组孕妇蜕膜组织中Ang-1、Ang-2及VEGF的表达显著低于对照1组,其差异有统计学意义(P<0.05)。观察组和对照组孕妇在人工流产次数、自然流产次数、不孕时间、体重指数、子宫内膜厚度、获卵泡数、窦卵泡数、胚胎移植数比较差异无统计学意义(P>0.05)。观察组孕妇的年龄、促排卵药物使用量以及卵泡生成激素/黄体生成激素比值均显著低于对照组,其差异有统计学意义(P<0.05)。进一步采用多因素Logistic回归分析对IVF-ET术后稽留流产危险因素分析发现,高龄、促排卵药物使用量大、卵泡生产激素/黄体生成激素比值大是孕妇IVF-ET术后稽留流产的危险因素(P<0.05)。结论 IVF-ET术后稽留流产孕妇蜕膜组织中Ang-1、Ang-2及VEGF的表达呈现低表达。导致IVF-ET术后稽留流产的原因是多方面的,特别是年龄、促排卵药物使用量及激素,因而根据年龄进行临床治疗,控制促排卵药物使用量以及密切关注激素水平变化,以避免流产情况。
Objective:To investigate the expression of angiopoietin-1(Ang-1),angiopoietin-1(Ang-2)and vascular endothelial growth factor(VEGF)in decidual tissue and in vitro fertilization embryo transfer(IVF)-ET)The relationship between missed abortion after surgery,and analysis of the influencing factors of missed abortion after IVF-ET,to provide a basis for prevention of abortion after IVF-ET. Methods:Eighty-two patients with missed abortion after IVF-ET admitted to our hospital from January 2016 to January 2018 were selected as the observation group of this study. 82 consecutive volunteers with abortion were selected as the control for this study. Group 1. The decidual tissues of the two groups were obtained after abortion and the expressions of Ang-1,Ang-2 and VEGF were measured. The expressions of Ang-1,Ang-2 and VEGF in the decidua of the two groups were compared. In addition,82 consecutive IVF-ET patients who continued to undergo pregnancy were selected as the control group 2 of the study. The clinical data of the two groups were compared. Logistic regression analysis was used to analyze the factors affecting abortion. Results:The expressions of Ang-1,Ang-2 and VEGF in the decidua of the observation group were significantly lower than those in the control group(P<0.05). There were no significant differences in the number of induced abortions,number of induced abortions,infertility time,body mass index,endometrial thickness,number of follicles,number of antral follicles,and number of embryo transfer between the observation group and the control group(P>0.05). The age of the pregnant women,the amount of ovulation-promoting drugs and the ratio of follicle stimulating hormone/luteinizing hormone were significantly lower in the observation group than in the control group,and the difference was statistically significant(P<0.05).Further analysis of risk factors for missed abortion after IVF-ET using multivariate logistic regression analysis found that high age,large amount of ovulation-promoting drugs,and large ratio of follicle production hormone/luteopoietin were risk factors for missed abortion after IVF-ET in pregnant women.(P<0.05). Conclusion:The expression of Ang-1,Ang-2 and VEGF in the decidual tissue of pregnant women with missed abortion after IVF-ET is low. The causes of missed abortion after IVF-ET are various,especially age,ovulation drug use and hormones,so clinical treatment according to age,control of ovulation drug use and close attention to changes in hormone levels to avoid miscarriage Happening.
