摘要
目的探讨妊娠期糖尿病前期与血清25(OH)D3水平的关系,为妊娠期糖尿病防治及其危险因素的研究提供新的思路。方法选择2017年1月至2018年1月在南通大学附属海安医院产科初诊并进行产前检查的妇女800例,入选妊娠期糖尿病前期348例,选取正常孕妇150例作为对照组。对2组孕妇的BMI、空腹血糖、服糖后1h血糖、服糖后2h血糖、空腹胰岛素及25(OH)D3等指标进行统计分析与比较。结果妊娠期糖尿病前期组在BMI、空腹血糖、服糖后2h血糖均高于对照组(P<0.01);妊娠期糖尿病前期组25(OH)D3明显低于对照组(P<0.01);两组在年龄、空腹胰岛素、服糖1h血糖上的差异无统计学意义(P>0.05)。血清25(OH)D3水平与服糖1h血糖及服糖2h血糖呈负相关(P<0.05),而与年龄、BMI、空腹血糖及空腹胰岛素无显著相关性(P>0.05)。25(OH)D3水平与妊娠期糖尿病前期呈负相关。结论维生素D水平可作为妊娠期糖尿病的预测指标,对预防妊娠期糖尿病的发生、发展具有重要意义。
Objective To explore the relationship between the gestational prediabetes and the level of serum 25(OH)D 3 ,so to provide a new insight for the prevention of gestational diabetes and its related risk factors. Methods 800 cases of women who underwent the initial diagnosis and antenatal examination in Nantong University affiliated Hai An Hospital were selected for the study from January,2017 to January,2018,including 348 qualified cases of gestational diabetes,and 150 cases of normal pregnant women as controls.Statistical analysis and comparisons were conducted among the pregnant women in the two groups on indexes,including BMI,fasting blood glucose,glucose after 1 hour of sugar-taking,glucose after 2 hours of sugar-taking and fasting insulin and 25(OH)D 3. Results The fasting blood glucose,glucose after 2 hours of sugar-taking of the gestational prediabetes group were all higher than those of the control group( P <0.01).The serum 25(OH)D 3 of the gestational prediabetes group was significantly lower than that of the control group( P <0.01).There were no significant differences in age,fasting insulin,and glucose after 1 hour of sugar-taking between the two groups( P >0.05).The serum 25(OH)D 3 level presented negative correlations with glucose after 1 hour of sugar-taking and glucose after 2 hours of sugar-taking,but no significant correlations with age,BMI,fasting glucose and fasting insulin( P >0.05).25(OH)D 3 level presented a negative correlation with gestational prediabetes. Conclusion Vitamin D level can serve as the forecast indicator and has an important significance for the prevention of gestational diabetes.
