摘要
目的分析不同胰岛素给药方式对妊娠期糖尿病患者妊娠结局的影响。方法 128例妊娠期糖尿病患者,按照胰岛素给药方法的不同分为治疗a组与治疗b组,每组64例。治疗a组予胰岛素泵持续注射治疗,治疗b组予胰岛素皮下注射治疗,观察并比较两组患者血糖水平(空腹血糖、餐后2 h血糖)及糖化血红蛋白水平,记录并比较两组患者胰岛素用量及不良妊娠结局(剖宫产、妊娠高血压、羊水过多、低血糖)发生情况。结果治疗后,治疗a组患者空腹血糖、餐后2 h血糖、糖化血红蛋白分别为(4.7±1.3)mmol/L、(6.8±1.5)mmol/L、(6.2±1.0)%,均低于治疗b组的(5.9±2.1)mmol/L、(8.7±2.3)mmol/L、(7.8±0.9)%,差异具有统计学意义(P<0.05);治疗a组胰岛素用量(44.2±4.7)U/d低于治疗b组的(52.3±6.4)U/d,不良妊娠结局发生率18.75%低于治疗b组的40.63%,差异均具有统计学意义(P<0.05)。结论妊娠期糖尿病应用胰岛素泵持续皮下注射安全性高,且能够帮助患者缩短血糖达标时间,有效改善妊娠结局。
Objective To analyze the effect of different insulin administration methods on pregnancy outcome of gestational diabetes mellitus patients. Methods A total of 128 gestational diabetes mellitus patients were divided by different insulin administration methods into treatment group a and treatment group b, with 64 cases in each group. Treatment group a received subcutaneous insulin pump injection and group b received subcutaneous insulin injection. The blood glucose (fasting blood glucose, postprandial 2 h blood glucose) levels and glycosylated hemoglobin in the two groups were observed and compared. The insulin dosage and occurrence of adverse pregnancy outcomes (cesarean section, pregnancy- induced hypertension, polyhydramnios, hypoglycemia) were recorded and compared between the two groups. Results After treatment, treatment group a had lower fasting blood glucose, postprandial 2 h blood glucose levels and glycosylated hemoglobin respectively as (4.7± 1.3) mmol/L,(6.8±1.5) mmol/L and (6.2±1.0)% than (5.9±2.1) mmol/L,(8.7±2.3) mmol/L and (7.8±0.9)% in treatment group b, and their difference was statistically significant (P<0.05). Treatment group a had lower insulin dosage as (44.2±4.7) U/d than (52.3±6.4) U/d in treatment group b, and lower incidence of adverse pregnancy outcomes as 18.75% than 40.63% in treatment group b, and the difference was statistically significant (P<0.05). Conclusion Continuous subcutaneous injection of insulin pump in gestational diabetes mellitus is safe and can help patients shorten the time of blood glucose reaching standard and effectively improve pregnancy outcomes.
