摘要
目的:分析神经内科不同能量肠内营养摄入对急性脑卒中患者耐受性的影响,为临床制定合理营养方案提供依据。方法:调查2017年3月1日~2018年1月31日进行肠内营养的急性脑卒中患者的病例资料,设低能量组68例,对照组40例。统计两组基础情况、用药情况。对比对照组和低能量组在营养耐受性指标、住院天数、死亡率、并发症等方面的差异。结果:两组在胃胀、呕吐、便秘和腹泻、应激性溃疡、消化道出血、死亡率、住院天数、肺部感染等方面无显著性差异。低能量组与对照组相比,机械通气率明显升高(14.70%vs. 2.50%,P=0.04),抗菌药使用天数明显延长(10天vs. 6天,P=0.04)。结论:两组在营养耐受性方面无显著差异,低能量组机械通气率升高,抗菌药使用天数延长,建议采用合理积极的营养方案,促进肠内营养合理应用。
Objective:To analysis the tolerance of different enteral nutrition in acute stroke patients to provide a clinical rational nutrition program.Methods:Acute stroke patients who received enteral nutrition were included from March 1,2017 to January 31,2018 in a neurology department.Basic information and indicators of clinical outcomes were reviewed.The differences in days of hospitalization,mortality,complications,nutritional indicators of tolerance,etc.were compared between the normal enteral nutrition group and the hypocalorie enteral nutrition group.Results:There were no significant differences in gastric retention,vomiting,constipation,diarrhea,stress ulcer,gastrointestinal hemorrhage,mortality,length of stay or incidence of pneumonia between the two groups.For the hypocalorie enteral nutrition group,compared with the normal enteral nutrition group,the mechanical ventilation rate was significantly higher(14.70%vs 2.50%,P=0.04)and the days of antibacterial use were significantly more(10 vs 6 days,P=0.04).Conclusion:There was no difference in enteral nutrition tolerance between the two groups while hypocalorie enteral nutrition leads to higher mechanical ventilation rate and longer antibacterial drug usage.It is recommended to adopt a rational and active nutrition program to promote rational application of enteral nutrition.
