摘要
目的观察醒脑静注射液穴位注射联合康复训练对智力低下患儿认知发育的影响。方法将80例智力低下患儿按照随机数字表法分为2组。对照组40例予康复训练治疗;治疗组40例在对照组治疗基础上联合醒脑静注射液穴位注射。2组均1个月为1个疗程,疗程间休息1周,共治疗4个疗程。比较2组治疗前后中医证候评分及听觉、认知、智力改善情况,并检测大脑中动脉血流指标[收缩期峰值流速(Vs)、舒张末期血流速度(Ved)、平均血流速度(Vm)及阻力指数(RI)],比较2组不良反应发生情况。结果治疗组治疗后筋骨痿弱,神情呆钝,坐起、站立、行走、生齿、语言迟缓,智力不健,疲倦乏力评分均降低(P <0. 05),且均低于对照组(P <0. 05),发稀疏萎黄评分无明显改善(P> 0. 05)。对照组治疗后仅筋骨萎弱,坐起、站立、行走、生齿、语言迟缓,疲倦乏力评分降低(P <0. 05),发稀疏萎黄、神情呆钝、智力不健评分与治疗前比较差异无统计学意义(P> 0. 05)。治疗后2组P300潜伏期缩短(P <0. 05),蒙特利尔认知评估量表(MoCA)评分、IQ评分均升高(P <0. 05);且治疗组P300潜伏期短于对照组(P <0. 05),MoCA评分及IQ评分均高于对照组(P <0. 05)。治疗后治疗组Vs、Ved、Vm均升高(P <0. 05),RI降低(P <0. 05);且治疗组Vs、Ved、Vm均高于对照组(P <0. 05),RI低于对照组(P <0. 05)。对照组治疗后Vm升高(P <0. 05),Vs、Ved及RI较治疗前无明显改善(P> 0. 05)。2组不良反应发生率比较差异无统计学意义(P> 0. 05)。结论醒脑静注射液穴位注射联合康复训练可有效缓解智力低下患儿临床症状,促进智力、听觉及认知发育,并改善脑循环,且无明显不良反应。
Objective To observe the clinical effects of acupoint injection of Xingnaojing injection combined with rehabilitation training on cognitive development of children with mental retardatio.Methods 80 children with mental retardation were divided into two groups according to the random number table method.40 cases in control group were treated by rehabilitation training.40 cases in treatment group were treated by acupoint injection of Xingnaojing injection on the basis of the control group.Both groups were treated for a course of one month,with a week's rest between the courses and a total of four courses of treatment.The scores of TCM syndromes and the improvement of hearing,cognition and intelligence were compared between the two groups before and after treatment.The middle cerebral artery blood flow indexes [systolic peak flow velocity (Vs),end-diastolic blood flow velocity (Ved),mean blood flow velocity (Vm) and resistance index (RI)] were measured,and the incidence of adverse reactions was compared between the two groups.Results After treatment,the scores of weakness of bones and muscles,sluggish look,sitting up,standing,walking,dentation,language retardation,mental retardation and fatigue in the treatment group were all decreased (P<0.05),and both were lower than the control group (P<0.05),there was no significant improvement in the hair sparsity and yellowness score (P>0.05).After treatment,the scores of weakness of bones and muscles,sitting up,standing,walking,dentation,language retardation,fatigue in the control group were decreased (P<0.05),the scores of hair sparsity and yeuowness,slnggish look and mental retardation were not significantly different from those before treatment (P>0.05).After treatment,the latency of P300 was shortened (P<0.05 ),the Montreal Cognitive Assessment Scale (MoCA) score and IQ score were increased (P<0.05),and the latency of P300 in the treatment group was shorter than that in the control group (P<0.05),the MoCA score and IQ score were higher than those in the control group (P<0.05).After treatment,Vs,Ved and Vm in the treatment group increased (P<0.05),RI decreased (P<0.05),and Vs,Ved and Vm in the treatment group were higher than those in the control group (P<0.05),RI was lower than that in the control group (P<0.05).After treatment,Vm in the control group increased (P<0.05),while the Vs ved and RZ did not improve significantly (P>0.05 ).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Acupoint injection combined with rehabilitation training can effectively alleviate the clinical symptoms of children with mental retardation,promote the development of intelligence,auditory and cognitive,and improve cerebral circulation without obvious adverse reactions.
