血清肌钙蛋白Ⅰ、肌酸激酶同工酶及乳酸转氨酶联合检测对小儿手足口病诊断意义探讨 被引量:14

摘要 目的探讨血清肌钙蛋白Ⅰ(cTnⅠ)、肌酸激酶同工酶(CK-MB)、乳酸转氨酶(LDH)联合检测对小儿手足口病(HFMD)的诊断意义。方法 2017年1月至2018年8月我院收治的HFMD患儿85例(观察组)、健康体检儿童40例(对照组),其中重症HFMD 32例,普通HFMD 53例。全自动生化分析仪检测血清LDH水平,化学发光分析仪检测血清CK-MB、cTnⅠ水平,比较各组血清CK-MB、LDH、cTnⅠ水平及阳性率,分析血清cTnⅠ、CK-MB、LDH水平的相关性。结果重症组血清cTnⅠ、CK-MB、LDH水平高于普通组与对照组(P <0. 05);观察组血清cTnⅠ、CK-MB、LDH阳性率及三者联合检测阳性率分别为75. 29%、56. 47%、43. 53%、89. 41%,均高于对照组(P <0. 05);血清cTnⅠ与CK-MB、LDH水平均呈正相关(P <0. 05)。结论血清cTnⅠ、CK-MB及LDH联合检测对小儿HFMD病情程度及心肌损害评估有重要意义。 Objective To investigate the diagnostic significance of combined detection of serum cardiac troponin Ⅰ( cTnⅠ),creatine kinase isoenzyme( CK-MB) and lactate transaminase( LDH) for infantile hand-foot-mouth disease( HFMD). Methods Eighty-five children with HFMD( observation group) admitted during the period from January 2017 to August 2018 and 40 healthy children undergoing physical examination( control group) were selected.Of the sick children,there were 32 cases with severe HFMD and53 cases with normal HFMD.Serum levels of CK-MB and LDH were detected by an automatic biochemical analyzer,and the serum cTnⅠ level was detected with a chemiluminescence analyzer.Serum levels and positive rates of CK-MB,LDH and cTnⅠ were compared between the two groups.Pearson correlation analysis was performed to analyze the correlation among levels of cTnⅠ,CK-MB and LDH.Results The levels of serum cTnⅠ,CK-MB and LDH were significantly higher in the severe HFMD group than those in the normal HFMD group and the control group( P < 0. 05).The positive rates of serum cTnⅠ,CK-MB and LDH and the positive rate of combined detection of the three in the observation group were 75. 29%,56. 47%,43. 53% and 89. 41%,respectively,which were significantly higher than those in the control group( P < 0. 05).The serum cTnⅠ level was positively correlated with levels of CK-MB and LDH( P < 0. 05).Conclusion The combined detection of serum cTnⅠ,CK-MB and LDH has a great significance in assessing the severity of infantile HFMD and myocardial damage.
