摘要
目的探讨Solitaire AB支架机械取栓治疗急性脑梗死的围手术期护理经验和方法。方法选取2017年9月—2018年9月就诊于我院的50例急性脑梗死患者作为研究对象所有患者均实施Solitaire AB支架机械取栓治疗,从术前、术中与术后护理等总结护理经验。记录患者血管再通率、并发症发生情况、神经功能损伤程度。结果 50例急性脑梗死患者开通术中、术后再闭塞各3例,颅内出血转化5例,死亡3个,改良脑梗死溶栓试验分级(mTICI)≥Ⅱb为血管再通,成功再通42例,无压力性损伤、穿刺部位血肿等并发症发生;治疗3周后NIHSS评分低于治疗前,差异具有统计学意义(P <0.05)。结论术前密切监测患者生命体征、导尿、心理疏导,术中配合医生对患者护理,术后病情检测、穿刺部位护理、饮食指导、卧床护理以及出院指导等是Solitaire AB支架机械取栓治疗急性脑梗死的护理重点。
Objective To investigate the perioperative nursing experience and methods of Solitaire AB stent mechanical thrombectomy for acute cerebral infarction. Methods 50 cases of patients with acute cerebral infarction treated in our hospital from September 2017 to September 2018 were selected as the study subjects. All patients received mechanical thrombectomy with Solitaire AB stent. Nursing experience was summarized from preoperative, intraoperative and postoperative nursing. The vascular recanalization rate, complications and neurological impairment were recorded. Results In 50 patients with acute cerebral infarction, there were 3 re-occlusions during and after operation, 5 intracranial hemorrhage transformations and 3 deaths. The modified thrombolytic test grade(mTICI)>IIb was recanalization. 42 cases were successful recanalization. There were no complications such as pressure injury, puncture site hematoma and so on. After 3 weeks of treatment, NIHSS score was lower than that before treatment, and the difference was statistically significant(P < 0.05). Conclusion Preoperative close monitoring of vital signs, urinary catheterization, psychological counseling, intraoperative cooperation with doctors in patient care, postoperative condition detection, puncture site care, dietary guidance, bed-rest care and discharge guidance are the key points of nursing Solitaire AB stent mechanical thrombectomy for acute cerebral infarction.
