摘要
目的研究免疫因子γ-干扰素(Interferon-γ,IFN-γ)及白细胞介素-4(Interleukin-4,IL-4)在高危型人乳头状瘤病毒(Human papillomavirus,HPV)感染的宫颈病变患者血清中浓度水平的变化,初步探讨其与疾病发生、发展的关系。方法选取高危型HPV感染的宫颈病变患者:宫颈癌患者42例(I期14例、Ⅱ期20例、Ⅲ期8例),宫颈上皮内瘤变者(Cervical intraepithelial neoplasia,CIN)53例(CINⅠ级25例、CINⅡ级12例、CINⅢ级16例),宫颈炎者40例,对照组39例。采用酶联免疫吸附技术检测其血清中IFN-γ、IL-4的浓度水平。结果 IFN-γ和IL-4在宫颈癌、宫颈上皮内瘤变、宫颈炎及对照组中的浓度随着宫颈病变级别的进展而变化:IFN-γ浓度逐渐降低,其中宫颈癌患者血清中IFN-γ浓度明显低于对照组及宫颈炎组(P<0.05);CINⅢ组患者血清中IFN-γ浓度与对照组、宫颈炎组、CINⅠ组及CINⅡ组比较差异无统计学意义(P>0.05);IL-4浓度随着宫颈病变级别的进展而逐渐升高,其中宫颈癌患者血清中IL-4浓度明显高于对照组、宫颈炎组及CIN组(P<0.05);CINⅢ组中IL-4浓度与对照组比较,差异有统计学意义(P<0.05);对照组、宫颈炎组、CINⅠ组和CINⅡ组之间,IL-4的浓度差异无统计学意义(P>0.05)。结论 IFN-γ和IL-4在宫颈癌、宫颈上皮内瘤变、宫颈炎中的浓度随宫颈病变级别的进展而变化,IFN-γ及IL-4可能共同参与了宫颈病变的发生机制。
Objective To study the change of interferon-γ(IFN-γ) and interleukin-4(IL-4) in patients with high risk type human papillomavirus(HPV) infection and explore the relationship between them and the occurrence,development of disease. Methods Selected high-risk type HPV infection of patients with cervical lesions:42 patients with cervical carcinoma,53 cases with cervical intraepithelial neoplasia(CINⅠ 25 cases,CINⅡ 12 cases,CINⅢ 16 cases),40 cases with cervicitis,39 normal cases. Used enzyme-linked immunosorbent technique to detect the serum IFN-γ,IL-4 levels. Results The concentration change of IFN-γ and IL-4 with the progress of the cervical lesion level:the concentrations of IFN-γ in cervical cancer patients was lower than the control group and cervicitis group(P<0.05);there were no statistically significant differences in CIN Ⅲ group and control group,cervicitis group,CINⅠgroup and CINⅡ group(P>0.05). The concentration of IL-4 increased with the progress of the level of cervical lesions,the concentration of IL-4 cervical cancer patients was higher than those in control group,cervicitis groups and CIN groups(P<0.05);compared with control group,the difference of IL-4 concentration in CIN Ⅲ group was statistically significant(P<0.05);there were no statistically significant differences in control group,cervicitis groups,CINⅠgroup and CINⅡgroup (P>0.05). Conclusion The changes of IFN-γ and IL-4 with the progress of cervical lesions(cervical cancer,cervical intraepithelial neoplasia,cervicitis and normal),IFN-γ and IL-4 may be involved in the pathogenesis of cervical lesions.
