摘要
糖尿病性视网膜病变是临床常见的致盲性眼病,其发病率有逐渐升高的趋势,现代医学对其发病机制尚不十分明确,目前尚无十分有效的治疗方法,且治疗中风险与疗效并存,如何有效治疗糖尿病性视网膜病变已成为研究热点。现代医学对其发生机制进行了初步的开展,主要从血糖水平及糖尿病病程、高血压及高血脂、多元醇代谢通路的异常、蛋白质非酶糖化产物的堆积、氧化应激反应增强、血管紧张素转换酶系统、细胞因子等角度进行研究。目前西医学对糖尿病性视网膜病变的治疗主要有3个方面：激光治疗、玻璃体腔注射、玻璃体切除术,近年来也有学者发现ACEI类及中药制剂离子导入法等治疗对糖尿病性视网膜病变亦有一定的疗效。中医学通过辨证论治,对糖尿病性视网膜病变的治疗取得了良好的疗效,在一定程度上帮助患者改善了视力,提高了生活质量。该文通过阐述糖尿病性视网膜病变的现代发病机制、现代医学对其的治疗措施,中医学对其病因病机的独特见解及治法治则,期望为糖尿病性视网膜病变的治疗提供一定的帮助。
Diabetic retinopathy is a common cause of blindness,its incidence increased gradually,in modern medicine,the pathogenesis is not very clear,there is no effective treatment method and curative effect in the treatment and risk coexist,how to effectively treat retinopathy of diabetes has become a hot research topic. Modern medicine has carried on the preliminary development of the mechanism,mainly from the level of blood glucose and duration of diabetes,hypertension and high blood lipids,polyol pathway abnormalities,protein non enzymatic glycation,oxidative stress,enhanced accumulation of angiotensinconverting enzyme system,cytokines and other perspectives. At present the treatment of Western medicine on diabetic retinopathy has three main aspects：laser therapy,intravitreal injection,vitreous body resection,in recent years,scholars have also found ACEI and preparation of traditional Chinese medicine iontophoresis treatment has a curative effect on diabetic retinopathy. Traditional Chinese medicine has achieved good results in the treatment of diabetic retinopathy through differentiation and treatment. To some extent,it helps patients improve their vision and quality of life. This article expounds the modern pathogenesis of diabetic retinopathy,the treatment measures of modern medicine,the unique insights and therapeutic principles of TCM on its etiology and pathogenesis,hoping to provide some help for the treatment of diabetic retinopathy.
