摘要
目的探讨保乳根治术(RMC)联合术中放疗(IORT)治疗乳腺癌(BC)的临床疗效。方法选择接受治疗的106例女性BC患者,根据计算机生成的随机数字分为IORT组(n=53)与术后放疗(PORT)组(n=53),IORT组行RMC联合IORT治疗,PORT组行RMC联合PORT治疗,观察两组手术相关指标、术后化疗开始时间、术后并发症发生情况、术后1年乳房外形评级情况、5年无病生存率及复发率。结果 IORT组患者手术时间、切口愈合时间均明显长于PORT组,术中出血量明显多于PORT组,差异均有统计学意义(P﹤0.01);两组患者术后负压引流量、住院时间、术后化疗开始时间比较,差异均无统计学意义(P﹥0.05)。两组患者术后并发症总发生率、术后切口Ⅰ期愈合率、术后1年乳房外形评级优良率比较,差异均无统计学意义(P﹥0.05)。两组患者5年同侧复发率、远处复发率比较,差异均无统计学意义(P﹥0.05);IORT组患者5年无病生存率高于PORT组,差异有统计学意义(P﹤0.05)。结论 RMC联合IORT治疗早期BC可有效杀灭乳腺癌细胞,术后无病生存率高,术后乳房美容效果好,值得应用于临床。
Objective To investigate the clinical value of radical mastectomy conserving（RMC） combined with intraoperative radiotherapy（IORT） in the treatment of breast cancer（BC）. Method One hundred and six female BC patients who chose to be treated were divided into IORT group（n=53） and PORT group（n=53） according to computer-generated random numbers. Patients in IORT group were treated by RMC combined with IORT, and patients in PORT group were treated with RMC combined with PORT. The surgical related indicators, postoperative chemotherapy starting time, postoperative complication, postoperative breast shape rating after 1 year, 5-year disease-free survival rate and recurrence rate of the two groups were observed. Result The operation time and incision healing time in the IORT group were significantly longer than those in the PORT group, the intraoperative blood loss in the IORT was more than that in the PORT group（P〈0.01）. There was no significant difference in the postoperative negative pressure drainage, hospitalization time,the starting time of postoperative chemotherapy between the two groups（P〉0.05）. There was no significant difference in the incidence of postoperative complications, the rate of healing in the first stage of the incision and the rate of breast shape evaluation in the first year after operation between the two groups（P〉0.05）. There was no significant difference in the 5-year ipsilateral recurrence rate and distant recurrence rate between the two groups（P〉0.05）. The 5-year disease-free survival rate of the IORT group was higher than that of the PORT group（P〈0.05）. Conclusion RMC combined with IORT can effectively kill breast cancer cells in BC, with high postoperative disease-free survival rate, good postoperative breast cosmetic effect, and is worthy of clinical application.
