摘要
目的总结应用浅表静脉引流腹壁下动脉穿支皮瓣(DIEP)乳房再造术的手术方法,探讨分析其适应证及优点。方法精细解剖浅表腹部静脉系统,并游离腹部浅表静脉及腹壁下动、静脉穿支,形成带有浅表静脉的腹壁下动脉穿支蒂皮瓣,将腹壁下动、静脉与患者胸廓内血管相吻合,浅表静脉与胸骨旁穿支血管静脉相吻合,进行乳房再造。结果自2016年起莫斯科国立谢东诺夫第一医科大学第一附属医院整形与重建外科此种术式临床应用35例,所有皮瓣全部存活,再造乳房外形满意。结论应用浅静脉吻合胸骨旁穿支血管静脉引流,可有效防止皮瓣灌注不足及静脉危象造成的危险并发症,使皮瓣成活率提高,达到手术最终目的。
Objective To summarize the surgical methods of superficial vein drainage for the treatment of the deep inferior epigastric perforator flap(DIEP)breast reconstruction,and to analyze its indications and advantages.Method Delicately dissect the abdominal superficial venous system,dissociate abdominal superficial vein and inferior epigastric artery and venous perforator,formation of the inferior epigastric perforator flap with superficial vein,the inferior epigastric artery and vein are matched with the blood vessels in the thoracic cavity of the patient,and the superficial vein superdrainage through the parasternal perforator vessel for breast reconstruction.Results Since 2016,35 cases of surgical procedures have been applied to the department of plastic and reconstruction surgery of the First Affiliated Hospital of I.M.Sechenov First Moscow State Medical University,all of the flaps survived,and the reconstructed breasts were well-shaped.Conclusions This method of superficial vein drainage prevents dangerous perfusionrelated complications,the survival rate of the flap is increased to achieve the ultimate goal of the operation.
