摘要
目的:探讨高频超声及经直肠腔内超声对肛瘘术前诊断中的临床应用价值。方法:选取2016年7月-2018年2月期间本院收治的48例肛瘘患者作为研究对象,所有患者在术前均采用传统直肠指诊和高频超声及经直肠腔内超声检查,并与手术病理结果对照,分析比较高频超声及经直肠腔内超声和传统直肠指诊的准确率。结果:超声诊断与直肠指诊诊断皮下瘘和括约肌间瘘的准确性比较,差异均无统计学意义(P>0.05);但高频超声及经直肠腔内超声诊断高位瘘的准确性高于直肠指诊,差异有统计学意义(P<0.05)。超声诊断肛瘘内口、支瘘数目以及合并瘘道脓肿的准确性分别为91.67%、92.59%、93.33%,直肠指诊诊断肛瘘内口、支瘘数目以及合并瘘道脓肿的准确性分别为70.83%、70.37%、33.33%,超声诊断的准确性高于直肠指诊,差异均有统计学意义(P<0.05)。结论:术前采用高频超声及经直肠腔内超声进行诊断,能提高肛瘘诊断率,还能准确显示出肛瘘的瘘管位置、走行、支管、内口以及其与括约肌的关系,为手术方案的制定提供影像学参考,具有较高的临床应用价值。
Objective：To evaluate the value of high frequency ultrasound and transrectal ultrasound in preoperative diagnosis of anal fistula.Method：From July 2016 to February 2018,48 patients with anal fistula were selected as the objects of study.All patients were examined with traditional digital rectal examination, high frequency ultrasound and transrectal ultrasound before operation.The accuracy of high frequency ultrasound,transrectal ultrasound and traditional digital rectal diagnosis were analyzed and compared with the results of operation and pathology.Result：There was no significant difference in the accuracy of the diagnosis of subcutaneous fistula and sphincter fistula between ultrasound and digital rectal diagnosis（P〈0.05）,but the accuracy of high-frequency ultrasound and transrectal ultrasound in the diagnosis of high-position fistula was higher than that in rectal finger diagnosis.The difference was statistically significant（P〈0.05）.The accuracies of ultrasonography in the diagnosis of anal fistula,number of branch fistula and complicated fistula abscess were 91.67%,92.59% and 93.33%respectively, which those in the rectal finger diagnosis were 70.83 % ,70.37% and 33.33% respectively.The accuracies of ultrasonography diagnosis were higher than that in rectal finger diagnosis,the differences were statistically significant（P〈0.05）.Conclusion：Preoperative diagnosis with high frequency ultrasound and transrectal ultrasound can improve the diagnosis rate of anal fistula, and can also accurately show the fistula position,direction, branch, internal mouth and its relationship with sphincter.It has high clinical application value to provide the imaging reference materials for the operation plan.
