摘要
目的:探讨白细胞(WBC)、超敏C-反应蛋白(hs-CRP)、降钙素原(PCT)联合检测对于新生儿胎粪吸入综合征(MAS)的诊断价值。方法:选取60例胎粪吸入综合征患儿和60例正常新生儿,比较MAS患儿和正常新生儿WBC、hs-CRP及PCT含量的差异;分析MAS患儿白细胞(中性粒细胞、嗜酸性粒细胞、嗜碱性粒细胞、单核细胞、淋巴细胞)检查异常的差异;单项指标检测和联合检测诊断MAS差异及灵敏度、特异度。结果:MAS患儿与正常新生儿WBC(t=13. 46,P=0. 00)、hs-CRP(t=20. 18,P=0. 00)及PCT(t=25. 90,P=0. 00)含量的差异均有统计学意义(均有P <0. 05); MAS患儿中性粒细胞、嗜酸性粒细胞、嗜碱性粒细胞、单核细胞、淋巴细胞异常率的差异有统计学意义(χ~2=27. 37,P=0. 00);单项指标WBC、PCT、hs-CRP和三项指标联合对疾病检出率的差异有统计学意义(χ~2=18. 46,P=0. 00);三项指标联合检测的灵敏度(0. 843)和特异度(0. 939)均达到最高。结论:WBC、PCT、hs-CRP对于新生儿胎粪吸入综合征的检出均有一定临床价值,三个指标的联合检测有助于疾病的明确诊断。
Objective To investigate the diagnostic value of combined detection of WBC,hs -CRP and PCT in neonatal meconium aspiration syndrome (MAS). Method The levels of WBC, hs -CRP and PCT in MAS children and normal newborns were compared between 60 children with meeoniunl aspiration syndrome and 60 normal newborns in our hospital. The leukoeytes ( neutrophils,Eosinophils,basophils,monoeytes, and lymphoeytes). The differences of sensitivity, specificity and specificity of single marker and combined detection were compared. Results WBC ( t = 13.46, P = 0. 00), hs - CRP ( t = 20. 18, P = 0. 00) and PCT (t = 25.90, P = 0. 00) in MAS and normal neonates were statistically significant (P 〈 0. 05 ). There was significant difference in the abnormal rate of neutrophils, eosinophils, basophils, monoeytes and lymphoeytes between the two groups (x^2 = 27.37, P = 0. 00). The sensitivity, specificity and specificity of the combined detection of three indicators (WBC ,PCT, hs -CRP and three indicators) were statistically significant (x^2 = 18.46, P = 0. 00) ; the sensitivity (0. 843 ) and specificity (0. 939 ) of the three indexes were the highest. Conclusion WBC, PCT, hs - CRP have certain clinical value for the detection of neonatal meeonium aspiration syndrome, but the combined detection of 3 indicators can help to eonfirm the diagnosis of the disease.
