摘要
目的分析血清3-羧-4-甲-5-丙基呋喃戊酮酸(CMPF)水平与T2DM患者胰岛β细胞功能的关系。方法选取2015年9月至2017年8月于上海市奉贤区中心医院内分泌科住院的血糖控制不佳T2DM患者453例,按照血清CMPF浓度由低到高平均分为Tertile 1组、Tertile 2组和Tertile 3组,每组151例。以C-P相关指标代表胰岛β细胞功能,对所有患者的资料进行分析。结果随着血清CMPF浓度升高,餐前、餐后C-P分泌减少。相关分析显示,血清CMPF水平与FC-P、0.5hC-P、2hC-P、OGTT C-P曲线下面积(AUC)、餐后0.5hC-P增值(Δ0.5hC-P)、餐后2hC-P增值(Δ2hC-P)、Δ0.5hC-P/PG、Δ2hC-P/PG均呈负相关(r=-0.272、-0.240、-0.208、-0.233、-0.210、-0.133、-0.208、-0.161,P<0.001,P=0.005)。多元线性回归分析显示,校正年龄、BMI、AST、ALT、eGFR后,0.5hC-P是血清CMPF的影响因素(β=-57.47,P=0.001)。结论血清CMPF水平升高是T2DM患者胰岛β细胞功能进行性减退的危险因素。
Objective To investigate the relationship between circulating 3-carboxy-4-methyl-5-propyl furanpropanoic acid(CMPF)level and isletβ-cell function in patients with type 2 diabetes(T2DM).Methods A total of 453 patients with T2DM were enrolled in this study from Department of Endocrinology and Metabolism,Jinzhou Medical University Affiliated Fengxian Central Hospital during Sep 2015 to Aug 2017.All the subjects were divided into three groups according to their serum CMPF concentrations from low to high:Tertile 1 group(n=151),Tertile 2 group(n=151)and Tertile 3 group(n=151).The function of isletβ-cell was represented by C-peptide related index,the relationship of CMPF levels and isletβ-cell function were analyzed. Results With the increase of serum CMPF concentration,the secretion of C-P before and after meal decreased.Correlation analysis showed that serum CMPF level was negatively correlated with FC-P,0.5 hC-P,2 hC-P,OGTT C-P area under curve(AUC),0.5 hC-P value after meal(Δ0.5 hC-P),2 hC-P value after meal(Δ2 hC-P),Δ0.5 hC-P/PG,Δ2 hC-P/PG(r=-0.272,-0.240,-0.208,-0.233,-0.210,-0.133,-0.208,-0.161,P0.001,P=0.005).Multivariate linear regression analysis showed that after adjusting for age,BMI,AST,ALT and eGFR,serum CMPF was negatively correlated with 0.5 hC-P(β=-57.47,P=0.001). Conclusion Increasing circulating CMPF level is a risk factor for progressive isletβ-cell dysfunction in patients with T2DM.
