摘要
目的：研究痤疮凹陷性瘢痕治疗中积雪苷霜软膏联合点阵CO_2激光对临床疗效的影响。方法：78例痤疮凹陷性瘢痕患者,按随机数字表法分为对照组（n=38例）和研究组（n=40例）。其中对照组患者采用点阵CO_2激光治疗,研究组患者采用点阵CO_2激光联合积雪苷霜软膏治疗。对比两组患者1次治疗后、两次治疗后ECCA评分及疗效,误工期及不良反应。结果：对照组患者治疗前ECCA评分为（69.48±18.31）分,一次治疗后为（50.48±16.48）分,一次治疗后ECCA评分与治疗前ECCA评分对比有统计学差异（P〈0.05）,两次治疗后为（38.73±15.53）分,两次治疗后ECCA评分与一次治疗后ECCA评分对比有统计学差异（P〈0.05）。研究组患者治疗前ECCA评分为（68.27±20.58）分,一次治疗后为（55.52±12.90）分,一次治疗后ECCA评分与治疗前ECCA评分对比有统计学差异（P〈0.05）,两次治疗后为（39.03±10.05）分,两次治疗后ECCA评分与一次治疗后ECCA评分对比有统计学差异（P〈0.05）,这说明,对照组一次治疗后与治疗前、两次治疗后与一次治疗后的ECCA评分均明显下降。对照组患者两次治疗总有效率明显高于一次治疗总有效率（P〈0.05）,研究组患者两次治疗总有效率明显高于一次治疗总有效率（P〈0.05）。两组患者一次治疗后疗效对比无统计学差异（P〉0.05）,两组患者两次治疗后疗效对比也无统计学差异（P〉0.05）。对照组患者误工期为（10.60±3.87）d,研究组患者误工期为（7.91±3.37）d,两组患者误工期对比有统计学差异（P〈0.05）。两组患者不良反应比例对比无统计学差异（P〉0.05）。结论：积雪苷霜软膏联合CO_2激光可有效治疗痤疮凹陷性瘢痕,可缩短误工期,均未出现严重不良反应,说明点阵CO_2激光联合积雪苷霜软膏治疗痤疮凹陷性瘢痕安全有效,值得临床推广。
Objective： To research the clinical efficacy of centella triterpenes cream combined with fractional CO_2 laser in the treatment of atrophic acne scar. Methods： Seventy-eight atrophic acne scar patients were divided into control group and research group randomly. The control group was taken fractional CO_2 laser while the research group was treated by centella triterpenes cream combined with fractional CO_2 laser. We compared the ECCA score and clinical efficacy after one and two treatments,delay of working and adverse reflection of two groups. Results： The ECCA score of control group before treatment was（ 69. 48 ± 18. 31） and after one treatment was（ 50. 48 ± 16. 48）. There was statistical difference of ECCA score before and after one treatment（ P 〈0. 05）. After two treatments,the ECCA score was（ 38. 73 ± 15. 53） and there was statistical difference after one and two treatments（ P 〈0. 05）. The ECCA score of research group before treatment was（ 68. 27 ± 20. 58） and after one treatment was（ 55. 52 ± 12. 90）. There was statistical difference between the ECCA score before and after one treatment（ P 〈0. 05）. The ECCS score after two treatment was（ 39. 03 ± 10. 05）. Therewas statistical difference between after two and one treatments（ P〈 0. 05）. This showed that the ECCA score of one treatment vs. before treatment,two treatment vs. one treatment both had statistical difference. The total clinical efficacy of two treatments of control group was much higher than one treatment（ P〈 0. 05）. The total clinical efficacy of two treatments of research group was much higher than that of one treatment（ P〈 0. 05）. The clinical efficacy after one treatment of two groups had no statistical difference（ P 〉0. 05） and the clinical efficacy after two treatments of two groups had no statistical difference（ P〉 0. 05）. The delay of working of control group was（ 10. 60 ± 3. 87） d and that of research group was（ 7. 91 ± 3. 37） d. The delay of working of two groups had statistical difference（ P〈 0. 05）. The adverse reflection rate of two groups had no statistical difference（ P〉 0. 05）. Conclusion： The study confirmed the way of combing fractional CO_2 laser and Centella Triterpenes Cream to treat face atrophic acne scar is effective. The delays of working time are shortened and had no serious adverse reflection. This shows that the fractional CO_2 laser in the treatment of atrophic acne scars is safe and effective. It is worthy of promoting.
