摘要
目的探讨精益管理体系在手术室医院感染控制中的应用效果。方法遵循精益管理原理,从事-流程化、人-规范化、物-规格化3个层次明确事、人、物的职责,细化运作流程,形成有效的精益管理体系,对2013—2017年我院感染率、漏报率、抗生素使用率以及环境卫生情况进行分析,评价精益管理体系应用效果。结果 2013—2017年5年间感染率与漏报率情况呈下降趋势,抗生素使用率逐年降低,而抗生素合理使用率逐年上升。且在2017年环境卫生学监测指标合格率最高,在医护人员手、物体表面、使用中消毒液、空气、其他等环境卫生学监测指标合格率分别为94. 11%,97. 29%,95. 10%,83. 78%,84. 62%,明显高于2013年。结论精益管理体系有助于降低院内感染率及漏报率,提高抗生素有效利用率,改善院内环境卫生,保证临床护理安全。
Objective To investigate the application effect of lean management system in hospital infection control at operating room. Methods To follow the principles of lean management, we clarify the responsibilities of work, staff, and article according to three aspects, that is work shall follow process, staff shall follow standards and articles shall be normalized. By the above actions, the operational process was refined to form an effective lean management system. The infection rate, under-reporting rate, antibiotic use rate and environmental sanitation in our hospital from 2013 to 2017 were analyzed to evaluate the application effect of the lean management system. Results The infection rate and missing report rate showed a downward trend during the five years from 2013 to 2017. The use rate of antibiotics decreased year by year, and the rational use rate of antibiotics increased year by year. At the same time, the environmental hygiene monitoring index reached the highest qualified rate in 2017. The qualified rates of environmental hygiene monitoring indicators in the hands of medical personnel, surfaces of objects, disinfectants, air, and other aspects are 94.11%,97.29%,95.10%,83.78%, and 84.62% respectively, which was significantly higher than that in 2013. Conclusion Lean management system can help reduce the infection rate and false negative rate in hospital, and improve the effective utilization rate of antibiotics. In addition, it can improve the environmental sanitation in hospitals, and ensure the safety of clinical nursing.
