摘要
目的探讨带状疱疹患者临床症状严重程度与淋巴细胞以及免疫球蛋白相关指标的相关性,为带状疱疹的预后预测和诊治提供新的思路。方法选择2016年9月-2017年3月于皮肤科入院治疗的带状疱疹患者91例,根据临床症状分为轻中度组(41例)和重度组(50例),检测患者入院时各亚型T淋巴细胞百分比,对免疫球蛋白进行分型检测。结果重度患者CD4+/CD8+指标(2.46±0.83)高于轻中度患者(2.01±0.91)。重度带状疱疹患者Ig E含量(59.78±41.74)IU/m L低于轻中度患者(83.14±51.38)IU/m L,其余Ig A、Ig M、Ig G含量两组间差异均无统计学意义(P>0.05)。重度患者的Ig G2含量(4.40±1.93)g/L高于轻中度患者(3.59±1.52)g/L。探索Ig G亚型间比值关系,轻中度患者的ln(Ig G1/Ig G2)指标(0.787±0.341)高于重度患者(0.641±0.307),同时ln(Ig G3/Ig G4)指标(-0.146±0.102)高于重度患者(-0.274±0.115)。结论带状疱疹患者临床症状严重程度与淋巴细胞以及免疫球蛋白抗体之间存在一定的关系,积极探索更加敏感的指标鉴别诊断带状疱疹以及预测其预后,具有十分重要的意义。
Objective To explore the correlation between severity of clinical symptoms and lymphocyte cell or immunoglobulin-related indicators in patients with herpes zoster（HZ）,and to provide a new idea for prognosis prediction,diagnosis and treatment of HZMethodsWe selected 91 HZ inpatients who were treated in Department of Dermatology, from September 2016 to March 2017According to the clinical symptoms,the patients were divided into two groups：mild to moderate group（n=41） and severe group（n=50）The percentage of T lymphocytes of each subtype were detected at the time of admission and immunoglobin was subjected to genotyping assayResultsThe CD4＋/CD8＋ index of severe group（246±083） wass higher than mild to moderate group（201±091）The IgE index of severe group（5978±4174）IU/mL was lower than that in mild to moderate group（8314±5138）IU/mLThere was no significant difference in IgA,IgM and IgG index between the two groups（P〉005）The IgG2 index of severe group（440±193） g/L was higher than that in mild to moderate group（359±152）g/LThe ratio of IgG subtypes was investigatedThe index of ln（IgG1/IgG2） of mild to moderate group（0787±0341） was higher than that in severe group（0641±0307）In addition,the index of ln（IgG3/IgG4） was higher in mild to moderate group（-0146±0102） than in severe group（-0274±0115）.ConclusionThere are some correlations between severity of clinical symptoms and lymphocyte cells or immunoglobulin antibody in HZ patientsSome sensitive indexes are required for differential diagnosis and outcome prediction for HZ.
