常规超声及超声“萤火虫”成像技术诊断乳腺肿块微钙化的价值研究 被引量:15

The value of conventional ultrasound and ultrasound“firefly”imaging in the diagnosis of breast lumps microcalcification

摘要 目的研究常规超声及超声"萤火虫"成像技术对乳腺肿块微钙化的鉴别诊断价值。方法选取2015年1月-2016年12月在本院行乳腺超声检查的患者120例,均为女性,术前对每例患者行常规超声及超声"萤火虫"成像技术检查,对比两种检查方法对乳腺肿块微钙化的显示率;以病理学结果为诊断依据,评价超声诊断乳腺肿块的准确度、灵敏度、特异度的差异。结果 204个乳腺结节,"萤火虫"成像技术显示68个微钙化,常规超声显示37个微钙化,研究表明常规超声能检出的微钙化灶,在同一位置应用"萤火虫"成像技术均可显示,也有部分常规超声不能显示的钙化却能够在"萤火虫"图像中清晰可见,因此在检出肿块内微钙化方面,"萤火虫"成像技术明显优于常规超声,差异有统计学意义(χ~2=7.42,P<0.001)。尤其在恶性病例中检出微小钙化灶的阳性率(73.9%)明显高于常规超声(40.2%)。结论 "萤火虫"成像技术对乳腺恶性结节内的微小钙化显示较好。尤其当出现簇状分布的针尖样钙化,则更有利于超声明确肿块的良恶性。 Objective To investigate the diagnostic value of conventional ultrasound and ultrasound"firefly"imaging for breast mass microcalcification.Methods From January 2015 to December 2016,120 women patients of breast lumps were inspected with conventional ultrasound and ultrasound"firefly"imaging technology,comparing the microcalcification detection rate of the two kinds of methods for breast lumps;based on pathology results,the accuracy,sensitivity and specificity of ultrasonography in the diagnosis of breast masses were evaluated.Results There were 204 breast nodule,"firefly"imaging showed68 microcalcifications,conventional ultrasonic detection showed 37 microcalcifications.The study showed that microcalcification can be detected by conventional ultrasound,and application of "firefly"imaging technology in the same location can be displayed.There were also parts of calcification that can't be seen by conventional ultrasound but can be seen clearly in"firefly"images,thus in the detection of microcalcification in the mass,"firefly"imaging technique was superior to conventional ultrasound,and the difference was statistically significant(χ~2=7.42,P〈0.001).In malignant cases,the positive rate of microcalcification(73.9%)of"firefly"imaging was obviously higher than that of conventional ultrasound(40.2%),especially.Conclusion Ultrasonic"firefly"is better to show microcalcifications ability in the malignant nodules,especially when the cluster distribution of"needlepoint"calcification is more clearly conducive to the identification of benign and malignant tumors.
