摘要
目的探讨经尿道前列腺电切术(transurethral resection of prostate,TURP)后导尿管最佳的固定方法和牵引时间。方法便利抽样法选择2015年6月至2017年2月,上海市奉贤区中心医院泌尿外科收治的200例良性前列腺增生(benign prostatie hyperplasia,BPH)住院患者为研究对象,均行TURP,术后留置F20三腔气囊导尿管,按随机数字表法将其分为A、B、C、D 4组,采用纱布结固定法、腹股沟区固定法分别牵引6、12h,评价并比较4组患者术后的出血量、疼痛评分、尿道口溢血发生率、尿道狭窄发生率。结果 4组患者的术后出血量及尿道狭窄的发生率经比较,差异均无统计学意义(均P>0.05);疼痛评分及尿道口溢血的发生率经比较,差异均有统计学意义(均P<0.05)。结论 TURP后留置导尿管采用腹股沟区固定法牵引6h即可达到止血目的,且有利于减轻患者疼痛、提高患者术后舒适度。
Objective To explore the best fixation method and traction time of catheters after transurethrue resection of prostate（TURP）.Methods By convenience sampling method,200 patients who underwent TURP from June 2015 to February 2017 were selected and randomly divided into A,B,C,D groups.F20 triple lumen balloon catheter was indwelt postoperatively.Gauze knot fixation method and inguinal fixation method were used to keep the catheter traction for 6 hours and 12 hours,respectively.The bleeding volume,pain index,incidence of urethral orifice hemorrhage and urethral stricture after surgery were measured in 4 groups.Results There was no statistically significant difference in postoperative bleeding,urethral stricture among four groups（all P〉0.05）.The statistical significance of pain index and incidence of urethral orifice hemorrhage were found in four groups（all P〈0.05）.Conclusions Inguinal fixation of urethral catheter for 6 hours after TURP can reach the purpose of hemostasis,significantly reduce the pain index,urethral orifice hemorrhage and improve patient comfort after surgery.
