摘要
为探讨术前外周血中性粒细胞/淋巴细胞的比值(NLR)、血小板/淋巴细胞的比值(PLR)和淋巴细胞/单核细胞的比值(LMR)对局部进展期结直肠癌患者预后的影响,回顾性分析在我院行结直肠癌根治术的220例患者临床资料,并计算NLR、PLR和LMR。采用ROC曲线将患者分为高比值组和低比值组,比较2组患者的临床资料和预后。采用Cox回归模型分析,评价各指标对预后的影响。结果显示,高NLR组和高PLR组术后5年生存率分别为37.8%和49.4%明显低于低NLR组和低PLR组的83.1%和74.8%,高LMR组术后5年生存率为73.2%,明显高于低LMR组的50.0%,差异均具有统计学意义(P<0.05)。Cox回归分析结果显示,年龄、分化程度、术后是否辅助化疗、TNM分期、术前NLR、LMR和PLR是结直肠癌预后的相关因素,其中年龄、TNM分期、分化程度、术前NLR是结直肠癌的独立预后因素。结果表明,术前高NLR、高PLR及低LMR是局部进展期结肠癌预后的危险因素,其中术前高NLR是预后的独立危险因素。
This study was to explore the influence of the preoperative values of NLR（neutrophil/lympho- cyte ratio）, PLR（platelet/lymphocyte ratio） and LMR （lymphocyte/monocyte ratio） in peripheral blood on the prognosis of patients with locally advanced colorectal carcinoma.Authors retrospectively analysed the clinical data of 220 patients with colorectal carcinoma who had received radical procedure for their carcino- ma in author＇s hospital, calculated the values of NLR,PLR and LMR,and divided the patients into two groups：high ratio group（HRG） and low ratio group（LRG） according to ROC curve showed,then compared above materials and patients prognosis, followed by evaluated the influence of above indexes on prognosis through Cox regression analysis. As results, high NLR group and high PLR group at postoperation had 37.8% of and 49.4% of 5-year survival rate respectively which was significantly lower than that of low NLR group and of low PLR group（37.8% and 49.4% vs 83.1% and 74.8 %, P〈0.05） ;meanwhile the rate in high LMR group was significantly higher than in low LMR group（73.2% vs 50.0%, P〈0.05）;the re- sults of Cox regression analysis showed age, differentiation degree, with or without postoperative, TNM staging,and the preoperative values of NLR, LMR as well PLR were the relevant factors to patient＇s prog- nosis;Among these factors age,TNM stage, differentiation degree,and preoperative NLR value were inde- pendent prognosic factors of colorectal carcinoma.Results show that preoperative high NLR,high PLR and low LMR are the risk factors related to the prognosis of locally advanced colorectal carcinoma in which pre- ot）erative high NLR is independent one.
