摘要
目的：探讨升清养阴汤辅助治疗对糖尿病肾病患者肾功能及血清TGF-β1、MMP-9、TIMP-1水平以及MMP-9/TIMP-1比值的影响。方法：选择2014年6月—2016年6月医院收治的80例糖尿病肾病患者为研究对象,采用随机数字表法分为观察组和对照组各40例。对照组给予血糖控制、低蛋白饮食、调节血脂、控制血压及对症等常规治疗;观察组在对照组治疗基础上加用升清养阴汤治疗。两组患者均连续治疗3个月,对比分析两组患者治疗前后eGFR水平、eGFR异常率及血清TGF-β1、MMP-9、TIMP-1水平、MMP-9/TIMP-1比值变化,并对临床疗效进行评价。结果：治疗3个月后,观察组患者eGFR水平较治疗前明显升高,eGFR的异常检出率均较治疗前明显降低,差异有统计学意义（P〈0.05或P〈0.01）,而对照组治疗前后差异无统计学意义（P〉0.05）,观察组eGFR水平明显高于对照组,差异有统计学意义（P〈0.05）。两组患者血清TGF-β1、MMP-9、TIMP-1水平及MMP-9/TIMP-1比值均较治疗前明显改善,差异均有统计学意义（P〈0.05或P〈0.01）,而观察组各指标改善均明显优于对照组,差异均有统计学意义（P〈0.05）。在临床疗效方面,观察组显效率明显高于对照组,差异有统计学意义（P〈0.05）,总有效率亦高于对照组,但差异无统计学意义（P〉0.05）。结论：对于糖尿病肾病患者,在常规治疗基础上加用升清养阴汤治疗,能进一步改善患者肾功能及肾组织细胞外基质代谢,从而在一定程度上提高临床疗效,值得临床推广。
Objective：To investigate the impact of Shengqing Yangyin Decoction on renal function and serum TGF-betal,MMP-9 and TIMP-1 in diabetic nephropathy patients. Methods：Eighty cases of diabetic nephropathy patients in our hospital during 2014. 6—2016. 6 as the research objects were divided into observation group and control group by using the random number table method,each group 40 cases. The control group＇s patients were given conventional treatment such as blood sugar control,low protein diet,regulating blood lipid,blood pressure control and the symptomatic therapy,etc. The observation group＇s patients were given Shengqing Yangyin Decoction on the basis of the control group treatment.Two groups were continuously treated for 3 months. We contrastively analyzed the changes of the eGFR level,eGFR abnormal rate and serum TGF-betal,MMP-9 and TIMP-1 levels,MMP-9/TIMP-1 ratio change of the two groups patients before and after treatment,and evaluated the clinical efficacy. Results：Three months after treatment,the eGFR level of the observation group patients was significantly increased and the anomaly rate of eGFR was significantly decreased.The difference had statistical significance（P〈0. 05 or P〈0. 01） and there was no statistically significant difference of the control group patients before and after treatment（P〉0. 05）. The eGFR level of observation group patients was obviously higher than that of control group. The difference had statistical significance（P〈0. 05）. The serum TGF-betal,MMP-9 and TIMP-1 levels and MMP-9/TIMP-1 ratio of two groups patients were improved obviously and the difference had statistical significance（P〈0. 05 or P〈0. 05）. And the index improvements of the observation group patients were significantly better than those of the control group. The difference had statistical significance（P〈0. 05）. The significant efficiency of observation group was obviously higher than that of control group and the difference had statistical significance（P〈0. 05）. The total effective rate was also higher than that of the control group,but there was no statistically significant difference（P〉0. 05）. Conclusion：Patients with diabetic nephropathy treated with Shengqing Yangyin Decoction on the basis of conventional treatment can further improve patients＇ renal function and kidney tissue extracellular matrix metabolism and improve the clinical curative effect to a certain extent. It is worthy of clinical promotion.
