孕中晚期补充复合维生素制剂与早产发生风险的关系 被引量:3

Relationship between multi-vitamins supplementation during the second and third trimester and risk of preterm delivery

摘要 目的探索孕中晚期在正常饮食的基础上补充复合维生素制剂对母亲早产发生风险的影响。方法按孕中晚期是否补充复合维生素制剂,将研究对象(n=600)分为复合维生素组、对照组,比较两组孕妇早产发生率的差异,以分层卡方检验分析复合维生素补充对早产发生风险的影响程度;并初步探索了复合维生素补充影响早产发生风险的机制。结果复合维生素组孕妇早产发生率显著低于对照组(χ~2=26.636,P<0.01);孕中晚期补充复合维生素可降低早产发生风险(OR=0.246,95%CI:0.138~0.439);复合维生素组孕妇血红蛋白浓度高于对照组,而血小板计数低于对照组(t值分别为-3.964、3.552,均P<0.01)。结论孕中晚期复合维生素补充是早产的保护性因子;复合维生素可提升孕妇外周血液中的血红蛋白浓度、降低血小板计数,从而可能通过改善血液环境达到降低早产发生风险的作用。 Objective To find the effects of multi-vitamins supplementation on basis of normal diet during the second and third trimester of pregnancy on risk of preterm delivery. Methods Study subjects (n= 600) were divided into multi vitamins group and control group based on whether multi-vitamins were supplemented during the second and third trimester of pregnancy. Difference in preterm birth rate between two groups was compared. A layered chi-square test was performed to analyze the effects of multivitamins supplementation on risk factors of preterm delivery. Furthermore, mechanism of multi vitamins supplementation influencing risk factors of preterm birth was explored initially. Results Preterm birth rate in multi-vitamins group was significantly lower than that in the control group (χ^2=26.636, P〈0.01). Multi-vitamins supplemented during the second and third trimester could decrease the risk of preterm delivery (OR =0. 246, 95% CI : 0. 138- 0. 439). Compared with the control group, women in multi-vitamins group had higher hemoglobin concentration and lower blood platelet count (t value was -3. 964 and 3. 552, respectively, both P 〈0.01). Conclusion Multi-vitamins supplementation during the second and third trimester of pregnancy is protective factor of preterm delivery. Multi-vitamins can increase hemoglobin concentration in peripheral blood of pregnant women and reduce blood platelet count, which may reduce risk of premature birth through improving blood environment.
