摘要
目的 研究卵巢功能早衰患者骨质疏松与机体甲状旁腺素(PTH)、25 -羟基维生素D(25-OH-VD)和骨钙素( BGP)水平的相关性. 方法 选择2012年7月至2015年5月在安康市中心医院接受治疗的卵巢功能早衰合并骨质疏松患者100例,其中绝经年龄〈36岁者48例,绝经年龄36-39岁者52例,另选同期在进行健康体检的52例女性作为对照组. 对比各组PTH、25-OH-VD和BGP等指标水平,分析卵巢功能早衰患者PTH、25-OH-VD和BGP与腰椎( L2-4 )骨密度( BMD)的相关性.结果 绝经年龄〈36岁组与绝经年龄36-39 岁组的左髋部BMD、腰椎BMD均显著低于对照组( t值分别为7.200、7.337、7.178、5.199,均P〈0.05),绝经年龄〈36岁组与绝经年龄36-39岁组间比较左髋部BMD、腰椎BMD差异无统计学意义(t值分别为0.000、1.326,均P〉0.05). 绝经年龄〈36岁组与绝经年龄36-39岁组的PTH水平均显著高于对照组( t值分别为2.283、2.165,均P〈0.05),25-OH-VD水平均显著低于对照组(t值分别为8.340、8.247,均P〈0.05),而各组的BGP水平相比,差异无统计学意义(F=1.094,P〉0.05). 腰椎BMD与PTH呈显著负相关(R=-0.549,P〈0.05),而与25-OH-VD呈显著正相关(R=0.507,P〈0.05). 结论 卵巢功能早衰患者发生骨质疏松与机体PTH水平升高以及25-OH-VD水平降低密切相关.
Objective To study the correlation of osteoporosis in premature ovarian failure patients with parathyroid hormone ( PTH) , 25-hydroxy vitamin D (25-OH-VD) and bone gla protein ( BGP) levels.Methods Totally 100 patients receiving treatment of ovarian failure with osteoporosis in Ankang Central Hospital of Shaanxi Province from July 2012 to May 2015 were collected for research, including 48 cases of menopausal age 〈36 years old and 52 cases of menopausal age 36-39 years old.Another 52 cases of healthy women were used as control group.The correlation between PTH, 25-OH-VD, BGP and BMD of the lumbar vertebrae ( L2-4 ) were analyzed in ovarian failure patients with osteoporosis.Results The left hip BMD and lumbar BMD of the age 〈36 years old group and the age of 36-39 years old group were significantly lower than those of the control group ( t value was 7.200, 7.337, 7.178 and 5.199, respectively, all P〈0.05).Neither left hip BMD nor lumbar BMD showed significant difference between age 〈36 years old group and age 36-39 years old group (t value was 0.000 and 1.326, respectively, both P〉0.05).The levels of PTH in two groups were significantly higher than those of the control group (t value was 2.283 and 2.165, respectively, both P〈0.05), while the levels of 25-OH-VD were significantly lower (t value was 8.340 and 8.247, respectively, both P〈0.05).BGP did not show significant differences among three groups (F=1.094, P〉0.05).The lumbar BMD was negatively correlated with PTH level (R=-0.549,P〈0.05) but positively correlated with 25-OH-VD (R=0.507,P 〈0.05).Conclusion Osteoporosis in premature ovarian failure patients is closely associated with increased PTH and decreased 25-OH-VD levels.
