摘要
目的:运用磁共振波谱(MRS)观察滋肾益髓方对髓海不足型帕金森病(PD)患者黑质区神经元活性的影响,为滋肾益髓方治疗PD提供影像学证据。方法:纳入符合标准的PD患者46例,以抗PD西药治疗为基础,加用中药滋肾益髓方治疗3个月。于入组时、治疗3个月分别进行PD一般资料收集、中西医量表评定,通过MRS采集大脑黑质区NAA、Cr、Cho数值,计算出NAA/Cr、Cho/Cr比值。通过对比治疗前后上述数据变化,探究滋肾髓方治疗PD的影像学依据。结果:治疗后患者黑质区上述MRS数据较治疗前无明显下降;PD患者病轻侧NAA/Cr与UPDRS-Ⅱ+Ⅲ及PD-QOL-Ⅰ、PD-QOL-Ⅰ+Ⅱ呈负相关,病轻侧Cho/Cr与UPDRS-Ⅲ、UPDRS-Ⅱ+Ⅲ及PDQOL-Ⅰ、PD-QOL-Ⅰ+Ⅱ呈正相关。结论:滋肾益髓方可延缓PD患者黑质区神经元损害,保持神经元活性。
Objective： To observe the effects of Zishen Yisui Formula on the activity of neurons in substantia nigra of Parkinson＇s disease（PD） patients with deficiency of marrow-reservoir by using magnetic resonance spectroscopy（MRS） and provide Imaging evidence. Methods： Forty six patients with PD were enrolled in this study. They were treated with anti-PD medicine and traditional Chinese medicine Zisheng Yisui Formula for 3 months. The PD general data collection and the Chinese and western medicine questionnaire were implemented at before and after 3 months of treatment to collect NAA, Cr, Cho values in substantia nigra by MRS, and NAA/Cr, Cho/Cr ratio were calculated by comparing the data change before and after treatment, the imaging basis of Zisheng Yisui Formula for the treatment of PD was explored. Results： The MRS data of the substantia nigra were just as those before treatment. NAA/Cr in the light side of PD patients was negatively correlated with UPDRS-Ⅱ＋Ⅲ, PDQOL-Ⅰand PD-QOL-Ⅰ＋Ⅱ. Cho/Cr in the light side of PD patients was positively correlated with UPDRS-Ⅲ, UPDRS-Ⅱ＋Ⅲ, PD-QOL-Ⅰ and PD-QOL-Ⅰ＋Ⅱ. Conclusion： Zishen Yisui Formula can delay the damage of neurons in substantia nigra of PD patients and maintain the activity of neurons.
