摘要
骨科手术内植物术后感染在临床上是一个十分棘手的问题,一旦发生常非常严重,而且处理时间越晚,对周围正常组织破坏越大,对患者的生理和心理都会造成沉重的负担。故而骨科手术内植物感染的早期诊断变得尤为重要,具有重要的临床意义。本文综述了骨科手术内植物置入术后感染的传统诊断方法,如白细胞计数、血沉、C反应蛋白,以及近年来用于术后感染早期诊断的新方法,主要集中在核医学、分子生物学、新型血清标志物等领域,以18F-FDG PET/CT、聚合酶链式反应(polymerase chain reaction,PCR)、血清降钙素原、白细胞介素-6为代表。随着这些技术的发展,在未来联合应用这些新型诊断手段并且建立标准化骨科内植物术后感染早期诊断流程是一个值得探索的课题。
Implant-associated infection after orthopedic operation is a very intractable problem in clinical practice and once it happens the complications are usually quite serious. The later the implant-associated infection is diagnosed the worse the surrounding normal tissue would be destroyed, which would be a heavy burden to patients physically and mentally. It is thus quite important to diagnose earlier for implant-associated infection clinically. Here we summarized the conventional diagnostic methods, such as leucocyte count,erythrocyte sedimentation rate(ESR) and C-reactive protein, and some new diagnostic methods like 18F-FDG PET/CT, polymerase chain reaction(PCR), serum procalcitonin and interleukin-6 mainly in nuclear medicine, molecular biology and new serum marker fields for implant-associated infection. With the development of these technologies, the combination application of these new diagnostic methods and establishment of standard diagnostic procedure for orthopedic implant-associated infection will be worth exploring.
