摘要
目的探讨新生儿重症监护病房(NICU)患儿医院感染的季节分布,为掌握医院感染的高危因素提供依据。方法回顾性收集2015年1月-2016年12月医院3 494例NICU患儿为研究对象,通过医院感染实时监测系统收集患儿临床资料和医院感染发生情况,将患儿按出生体质量分为≤1 000g、1001~1500g、1501~2500g和>2 500g四组。采用SPSS17.0软件分析感染患儿总体特征及医院感染季节发病分布特点。结果 NICU患儿中有102例发生医院感染,感染发生率为2.92%;出生体质量≤1 000g的医院感染发生率为28.13%、出生体质量>2 500g的医院感染发生率最低为2.02%(χ~2=143.243,P<0.001),出生体质量≤1 000g的医院感染例次发生率最高为37.50%、出生体质量>2500g的医院感染例次发生率最低为2.32%(χ~2=184.818,P<0.001);在不同季节中,NICU患儿医院感染部位以血流感染为主,共27例次占23.48%(P=0.670),NICU患儿医院感染发生率秋季最高为3.89%(χ~2=7.985,P=0.046),秋季的例次感染发生率最高为4.20%(χ~2=8.580,P=0.035)。结论低出生体质量儿尤其出生体质量<1 500g的新生儿、一年四季中的秋季、新生儿院内获得性血流感染是NICU患儿医院感染防控的重点人群、重点时间、重点部位。
OBJECTIVE To investigate the seasonal distribution of nosocomial infections in neonatal intensive care unit (NICU), so as to provide evidence for mastering high-risk factors of neonatal nosocomial infections. METHODS A retrospective analysis was made on the overall characteristics of 3,494 hospitalized newborns and the seasonal distribution of n0socomial infections from Jan. 2015 to Dec.2016 through the real-time monitoring system for nosocomial infections. All the patients were divided into 4 groups according to weight of birth: ≤1000g, 1001 ≤1500g, 1501-2500g and 〉2 500g. SPSS17.0 software was used to analyze the general characteristics of infec- tive children and the seasonal distribution of nosocomial infections. RESULTS There were 102 cases of nosocomial infections in newborns in NICU,and the incidence rate was 2.92%. The nosocomial infection rate (28.13%) and nosocomial infection cases rate (37.50 ~) of neonates with birth weight 〈1000g were the highest. The nosocomial infection rate (2.02%) and nosocomial infection cases rate (2.32%) of neonates with birth weight 〉2500g were the lowest. The nosoeomial infection rate (3.89%) and nosocomial infection eases rate (4.20%) of neonates in au-tumn were the highest and the difference between different seasons was significant (P〈0.05). In four seasons, bloodstream infection (except CLABSI) cases occurred most frequently (27 cases), accounting for 23.48%. CONCLUSION Low birth weight children, especially birth weight less than 1500g newborns, fall in all seasons throughout the year and neonatal hospital acquired bloodstream infections in neonatal intensive care unit in children are the key population, key time and key site of neonatal nosocomial prevention and control.
