药物和物理治疗对正畸牙根吸收修复影响的研究现状

摘要 根尖外吸收是继发于正畸牙齿移动的常见临床并发症,轻度的吸收在停止加力后一段时间可自行修复,但中重度的牙根吸收临床上尚缺乏有效对策。为避免这些并发症的出现,国内外学者展开了大量探索。迄今为止,大量体外和动物实验研究对根吸收的修复提供了新的见解和方法。本文主要从药物和物理治疗两个角度总结了可影响牙根吸收修复的相关方法,所提及的方法中对牙根修复的作用原理基本为上调相关矿化因子比值,促进成牙骨质细胞生成,提高牙骨质矿化和沉积。其中,低强度脉冲和低水平激光在治疗过程中还可进一步修复正畸所造成的牙周组织损伤。但这些方法均处于实验阶段,由于牙根吸收的高发性和不可预测性,寻找牙根修复的最佳方法和临床应用,还需进一步研究。 External apical root resorption is a frequent clinical complication secondary to orthodontic tooth movement. Mild root resorption can be repaired by stopping force.However,there is no effective countermeasure for moderate and severe root resorption in clinic.In order to avoid these complications,domestic and foreign scholars have launched a great deal of exploration.Todata,a substantial number of in vitro and in vivo studies have supplied data that provides new insights into root resorption.The paper summarizes the relevant methods which can affect root resorption from medicine and physics aspests.Some action principles of repair are described in these methods.These action principles include increasing the ratio of related mineralization factors,promoting the producing of cementoblast,improving the mineralization of cementum and raising the deposition of cementoblast.The orthodontic can cause periodontal tissue injuries.Low-level laser therapy and low-intensity pulsed ultrasound,which are mentioned in the summarize,can also repair periodontal tissue injuries. However,these methods are in the experimental stage.Due to the high incidence rate and unpredictability of root resorption in the orthodontic,fnding the best method and clinical application of root resurfacing needs further study.
