摘要
目的:探讨小柴胡颗粒对感染性心肌炎患者T淋巴细胞亚群和心肌酶学的影响。方法:选取中国医科大学附属第一医院2015年1月—2016年10月治疗的感染性心肌炎患者104例,按随机、对照原则分对照组52例和研究组52例,对照组采用抗病毒、营养心肌及增强免疫治疗;研究组在对照组基础上采用小柴胡颗粒治疗,两组共治疗4周为1疗程,治疗前后测定心功能指标,采血测定T淋巴细胞亚群、心肌酶学以及肌钙蛋白Ⅰ、肌钙蛋白T水平,同时对比临床疗效和安全性。结果:与治疗前比较,两组治疗后CI、EF、CO、SV升高,CD3+、CD4+、CD8+以及NK升高,LDH、CK-MB、CK、α-HBDH降低,肌钙蛋白T、肌钙蛋白Ⅰ降低(P<0.05);与对照组比较,研究组治疗后CI、EF、CO、SV较高,CD3+、CD4+、CD8+以及NK较高,LDH、CK-MB、CK、α-HBDH较低,肌钙蛋白T、肌钙蛋白Ⅰ较低(P<0.05);对照组有效率75.00%低于研究组有效率90.39%(P<0.05);两组间不良反应发生率,无差异(P>0.05)。结论:小柴胡颗粒对院内感染性心肌炎患者疗效确切,有助于降低心肌损伤,增强免疫功能,且安全性高,值得广泛推广。
Objective: To investigate the effect of Xiaochaihu granule on T lymphocyte subsets and myocardial enzymes in patients with hospital-acquired infectious myocarditis. Methods: 104 patients with hospital-acquired infectious myocarditis were randomly divided into the control group( n = 52) and the study group( n = 52). The control group was treated with antiviral medicine,nutritional myocardium,and immunostimulant; on which basis,the study group was also treated with Xiaochaihu granules. The treatment course was four weeks. The levels of T lymphocyte subsets,myocardial enzymes and troponin Ⅰ and troponin T were measured before and after the treatment,and the clinical efficacy and medication safety were compared. Results: The levels of CI,EF,CO,SV,CD3^+,CD4^+,CD8^+and NK were increased after the treatment,whereas the levels of LDH,CK-MB,CK,α-HBDH,troponin T,and troponin T were decreased( P < 0. 05); of which the increasing levels and decreasing levels of the study group was more significantly than those of the control group( P < 0. 05). The effective rate of the control group was 75. 00% ,which was significantly lower than 90. 39% of the study group( P < 0. 05). There was no significant difference in the incidence of adverse reactions between the two groups( P > 0. 05). Conclusion: Xiaochaihu granule has definite effect on patients with hospital-acquired infectious myocarditis,which can help to reduce myocardial injury and enhance immune function,and the medication is safe and is worthy to be widely popularized.
