住院老年患者肌少症危险因素研究 被引量:8

A study of correlation between sarcopenia and associated variables in hospitalized elderly patients

摘要 目的探讨我院老年住院患者肌少症与年龄、性别、骨密度、骨质疏松等因素的相关性。方法选取我科室2015年9月至2016年9月住院患者100人,记录一般情况,采用双能X线法测量股骨头骨密度(h-BMD)、腰椎骨密度(l-BMD)及四肢骨骼肌含量(ASM),计算出相对四肢骨骼肌指数(RASM),通过6米步行测试和测量握力评价躯体功能,应用亚洲肌少症工作组(AWGS)诊断标准诊断及评估肌少症。采用Pearson及Spearman相关分析方法统计肌少症与上述因素的相关性,采用卡方检验验证肌少症与骨质疏松症的相关性。结果所有老年患者RASM均与年龄呈负相关(男性r=-0.506,女性r=-0.659,P均<0.01),男性RASM与BMI、l-BMD呈正相关(BMI:r=0.380,lBMD:r=0.295,P均<0.01),女性RASM与h-BMD、l-BMD呈正相关(h-BMD:r=0.682,P<0.05、l-BMD:r=0.695,P<0.01)。Logistic回归分析提示老年男女性患者年龄是肌少症发病的危险因素,高龄老年人l-BMD与肌少症明显相关,老年女性的骨骼肌减少是骨质疏松的发病危险因素。结论肌少症与年龄、BMI、BMD、骨质疏松均有一定相关性,对于老年住院患者来说,骨质疏松是肌少症的明确危险因素。 Objective To the relationship between sarcopenia and age,gender,bone mineral density (BMD) and os- teoporosis in hospitalized elderly patients. Methods All 100 patients,were selected from geriatrics department with our hospital in September 2015 to September 2016. The BMD (g/cm2) of lumbar spine and hip,the appendicular skeletal muscle mass (ASM),relative appendicular skeletal muscle index(RASM) , were detected by dual energy X-ray bone density measurement. The muscle strength was evaluated by grip strength. The physical function was assessed by 6 meters walking test. The sarcopenia was comprehensive diagnosed by AWGS criteria. The correlation between ASM, RASM, muscle strength and BMD was analyzed by Pearson and Spearman correlation analysis methods, and the correla- tion between sarcopenia and osteoporosis was analyzed by chi-square test. Results RASM had significantly negative correlation with age in both genders(male r=-0. 664,P〈0.01 female r=--0. 659 ,P〈0.01) ,had positive correlation with BMI and BMD (BMI:male r=0. 482 ,P〈0.01 ,female r=0. 295(P〈0.05),hip..male r=0. 561 ,P〈0.01 ,female r=0. 682,P〈0.01,1umber:male r=0. 520,P〈0.01 ,female r=0. 695,P〈0.01) ,had negative correlation with osteo- porosis(male r=-0. 458, P〈0.01, female r=--0. 623, P〈0.01). Multiple linear regression analysis indicates that for elderly male patients, age and femoral bone density (h-BMD) had positive correlation with sarcopenia, but for elderly fe- male patients BMI did. Conclusion For all elderly patients sarcopenia has correlated with age,and BMI,the BMD and osteoporosis. For elderly patients,osteoporosis is definitely a risk factor of sarcopenia.
