摘要
目的:探索和总结林丽珠教授治疗大肠癌的临床思维和用药规律。方法:通过收集整理林丽珠教授辨证治疗大肠癌的178张门诊处方,运用频数分析方法,分析病案中的药物和药对使用频次,并对统计结果进行总结和归纳。结果:大肠癌患者46例,共就诊178次,共用中药69味,用药频次共1642次,使用频次最高的药物包括:土鳖虫、桃仁、地榆、槐花、党参、白术、枳壳、半枝莲、木香等;使用频次最高的药对包括:土鳖虫与桃仁、地榆与槐花、党参与白术、柴胡与白芍、山慈菇与半枝莲、牡丹皮与白芷;证型出现频次最高为:瘀毒内结。结论:林丽珠教授对大肠癌的治疗上,以辨证论治为核心,重视辨证与辨病相结合,以祛瘀解毒法为基本法,注重攻补兼施、通利腑气,灵活运用药对,临床疗效显著。
Objective：To study and summarize the clinical thinking and the laws of medicine application in treating colorectal cancer of professor LIN Lizhu.Methods：Collect the medical records of 178 outpatients of professor LIN Lizhu and then apply the frequency analysis to summarize the laws of drugs and couple drugs application in treating colorectal cancer.Results：The collection of professor LIN Lizhu ambulatory patients with colorectal cancer included 46 cases,a total of 178 visits,69 kinds of Chinese medicine,drug use frequency 1642 times.The commonly-used single drugs included Tubiechong（Eupolyphaga）,Taoren（Peach Kernel）,Diyu（Sanguisorba Officinalis）,Huaihua（Flos Sophorae）,Dangshen（Codonopsis）,Baizhu（Bighead Atractylodes Rhizome）,Zhike（Fructus Aurantii）,Banzhilian（Scutellaria Barbata）,Muxiang（Costustoot）,etc.The commonly-used couple drugs include Tubiechong（Eupolyphaga） and Taoren（Peach Kernel）,Diyu（Sanguisorba Officinalis） and Huaihua（Flos Sophorae）,Dangshen（Codonopsis） and Baizhu（Bighead Atractylodes Rhizome）,Chaihu（Radix Bupleuri） and Baishao（Radix Paeoniae Alba）,Shancigu（Edible Tulip） and Banzhilian（Bighead Atractylodes Rhizome）,Mudanpi（Moutan Bark） and Baizhi（Radix Angelicae）.Syndrome pattern which commonly occurred was internal accumulation of stagnant toxin.Conclusion：Principle of removing toxic substances and dissipate stasis is the basic law of the treatment of colorectal cancer and the core is differential treatment.Professor LIN Lizhu pays attention to combine syndrome differentiation with disease differentiation,tonification with purgation in combination,which have achieved satisfactory clinical efficacy after applying couple drugs reasonably.
