摘要
目的探究细节护理干预对蛛网膜下腔出血患者头痛及负面情绪的影响。方法选取哈尔滨医科大学附属第一医院2014年1月~2016年1月收治的蛛网膜下腔出血患者90例,按照就诊时间顺序分为常规护理组（2014年1月~2015年1月）与细节护理干预组（2015年2月~2016年1月）,每组45例。比较两组患者住院时间、头痛次数、头痛持续天数、头痛程度、焦虑抑郁评分及生活质量评分。结果细节护理干预组较常规护理组住院时间缩短、头痛次数减少、头痛持续天数缩短、头痛程度减轻,差异有统计学意义（P〈0.05）。两组患者护理前焦虑自评量表（SAS）及抑郁自评量表（SDS）评分比较,差异无统计学意义（P〉0.05）。两组患者护理后与护理前比较,SAS及SDS评分均降低;护理后,细节护理干预组SAS及SDS评分均低于常规护理组,差异有统计学意义（P〈0.05）。细节护理干预组躯体功能、角色功能、情感功能、认知功能及社会功能评分较常规护理组高,差异有统计学意义（P〈0.05）。结论细节护理干预应用于蛛网膜下腔出血患者中较常规护理可有效缓解头痛、负面情绪,提高生活质量。
Objective To explore the influence of headache and negative emotions about details nursing intervention for patients with subarachnoid hemorrhage. Methods 90 cases with subarachnoid hemorrhage treated in our hospital from January 2014 to January 2016 were selected.All the patients were divided into routine nursing group（ January 2014 to January 2015） and details nursing intervention group（ February 2015 to January 2016） according to the clinic time,45 cases in each group. The hospitalized days duration, number of headache, headache, headache, negative emotions and quality life of between the two groups of patients were compared. Results The hospitalization time shortened and headaches was fewer, headache days shortened, headache degree relieved when the details nursing intervention group compared with the routine nursing group, with statistically significant difference（P〈0.05）. The SAS and SDS score of the two groups had no obvious difference before nursing（P〈0.05）. The SAS and SDS score of the two groups were decreased after nursing, the SAS and SDS score of details nursing intervention group was lower than those of routine nursing group,with statistically significant difference（P〈0.05）. The physical function, role function, emotional function,cognitive function and social function score of the details nursing intervention group were higher than those of routine nursing group, with statistically significant difference（P〈0.05）. Conclusion The details nursing intervention in patients with subarachnoid hemorrhage can relieve headaches effectively, and alleviate the negative mood, promote the quality of life,when compared with the routine nursing group.
