摘要
目的研究游离微小RNA-198(miR-198)对肺癌相关的恶性胸腔积液的诊断价值。方法采用实时荧光定量PCR方法检测54例肺癌恶性胸腔积液和35例良性胸腔积液中胸液游离miR-198的相对表达水平。结果肺癌恶性胸腔积液miR-198的表达量小于良性胸腔积液,差异有统计学意义(P〈0.01)。ROC曲线显示miR-198能够较好地区分良恶性胸腔积液,曲线下面积(AUC)为0.899(95%CI:0.822~0.950),优于CEA的0.774(95%CI:0.716~0.825)。当miR-198和CEA联合检测时,敏感度增高到85.7%,特异度为90.9%,AUC升高到0.920(95%CI:0.844~0.963)。游离miR-198表达水平与MPE患者年龄、性别、吸烟史、肿瘤类型、肿瘤转移位置以及CEA的水平无关(P〉0.05)。结论胸液游离miR-198可作为肺癌恶性胸腔积液诊断的分子标志物。
Objective To evaluate the diagnostic value of cell- free miR- 198 in the diagnosis of lung cancer- associated malignant pleural effusions( MPEs). Methods The cell- free levels of miR- 198 from 54 MPEs and 35 benign pleural effusions( BPEs) were determined using a real- time reverse transcription quantitative polymerase chain reaction. Results The expression level of miR- 198 was significantly lower in the patients with MPEs than the patients with BPEs,with the difference statistically significant( P〈0. 01). ROC curve showed that miR- 198 can better distinguish between benign and malignant pleural effusion,the area under the curve( AUC) was 0. 899( 95% CI: 0. 822- 0. 950),which was better than 0. 774( 95% CI:0. 716- 0. 825) of CEA. When miR- 198 and CEA were combined,the sensitivity increased to 85. 7%,the specificity was90. 9%,and the AUC increased to 0. 920( 95% CI: 0. 844- 0. 963). Free miR- 198 expression levels was not correlative to age,sex,smoking history,tumor type,tumor metastasis,and CEA levels in patients with MPE( P〉0. 05). Conclusion Cell- free miR- 198 in patients could be used as a new biomarker for the diagnosis of lung cancer- malignant pleural effusion.
