巴曲酶联合银杏达莫对突发性耳聋患者听力及血液流变学的影响 被引量:24

Effect of batroxobin combined with ginkgo dipyidamolum on hearing and hemorheology in patients with sudden hearing loss

摘要 目的:探讨巴曲酶联合银杏达莫对突发性耳聋(SHL)患者听力及血液流变学的影响.方法:选取2015年1月~2015年12月耳鼻喉科收治的80例 SHL 患者为研究对象,采用随机数字表法分为观察组和对照组,两组患者均给予巴曲酶注射液10 BU+0.9%氯化钠100 mL,静脉滴注,隔日1次,观察组患者在上述治疗的基础上加用银杏达莫注射液30 mL+5%葡萄糖250 mL 静脉滴注,1次/d,采集两组患者治疗前及治疗10 d 时空腹肘静脉血,使用血液流变仪测定血浆黏度(PV)、全血黏度[高切黏度(HS)、中切黏度(MS)、低切黏度(LS)];采用全自动凝血功能仪测定凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血酶原时间(ATPP)、纤维蛋白原(FIB)、血小板因子(PF);采用酶联免疫吸附法测定血清连接蛋白26(Connexin26)、连接蛋白30(Connexin30)水平;治疗前后听阈值采用 AC40听力测试仪测定.结果:两组治疗后 PV、HS、MS、LS 水平均较治疗前降低(P <0.05),观察组上述指标改善幅度优于对照组(P <0.05);两组治疗后 TT、PT、ATPP 较治疗前上升,FIB、PF 较治疗前降低(P <0.05),观察组治疗后上述指标改善程度优于对照组(P <0.05);两组治疗后 Connexin26、Connexin30水平均较治疗前明显升高(P <0.05),观察组治疗后 Connexin26、Connexin30水平明显高于对照组(P <0.05);观察组治疗后听阈值明显高于对照组(P <0.05).结论:巴曲酶联合银杏达莫可有效改善 SHL血液高凝状态,上调 Connexin26、Connexin30水平,提高 SHL 患者听力,疗效确切. Objective:To explore the effect of batroxobin in combined with ginkgo dipyidamolum on the hearing and he-morheology in patients with sudden hearing loss (SHL).Methods:A total of 80 patients with SHL who were admitted in our hospital from January,201 5 to December,201 5 in ENT Department were included in the study and randomized into the obser-vation group and the control group.The patients in the two groups were given batroxobin injection (10 BU)+ 0.9% NaCl (100 mL),ivdrip,once every other day.On this basis,the patients in the observation group were given additional ginkgo dipy-idamolum injection (30 mL)+ 5% glucose (250 mL),iv drip,1 time/d.The fasting elbow venous blood before treatment and 10 d after treatment in the two groups was collected.The blood rheometer was used to detect the plasma PV,HS,MS,and LS.The full automatic coagulation apparatus was used to detect TT,PT,ATPP,FIB,and PF.ELISA was used to detect the serum Connexin26 and Connexin30 levels.AC40 audio tester was used to measure the hearing threshold value before and after treatment.Results:PV,HS,MS,and LS levels after treatment in the two groups were significantly reduced when compared with before treatment (P 〈0.05),and the improved degree of the above indicators in the observation group was significantly superior to that in the control group (P 〈0.05).TT,PT,and ATPP after treatment in the two groups were significantly ele-vated,while FIB and PF were significantly reduced when compared with before treatment (P 〈0.05),and the improved degree of the above indicators in the observation group was significantly superior to that in the control group (P 〈0.05).Connexin26 and Connexin30 levels after treatment in the two groups were significantly elevated when compared with before treatment (P 〈0.05).Connexin26 and Connexin30 levels after treatment in the observation group were significantly higher than those in the control group (P 〈0.05).The hearing threshold value after treatment in the observation group was significantly higher than that in the control group (P 〈0.05).Conclusions:Batroxobin in combined with ginkgo dipyidamolum can effectively improve the blood hypercoagulation status in SHL patients,up regulate Connexin26 and Connexin30 levels,and improve the hearing, with an accurate efficacy.
