摘要
目的:系统评价针刺治疗感音神经性耳聋的疗效。方法:通过计算机检索中国知网(CNKI)、万方数据学术全文数据库(Wangfang)、维普中文期刊全文数据库(VIP)、中国生物医学文献服务系统(SinoMed)、PubMed五大数据库,检索建库至2023年7月1日关于针刺治疗SNHL的临床随机或半随机对照研究。采用Revman5.3软件对其临床有效性及改善电测听听力效应进行Meta数据分析。结果:所有文献经纳入与排除,纳入文献23篇,纳入病例2130例,治疗组(针刺)和对照组(西药)相比,有效率分别为928/1121(82.8%),600/1009(59.4%)。Meta分析结果显示:针刺治疗SNHL有明确疗效(OR=3.60,95%Cl(2.92,4.45),Z=11.92,P<0.00001),治疗组对于SNHL的电测听听力损失值的改善有明显的优势(MD=-2.08,95%Cl(-3.40,-0.76),Z=11.92,P=0.002)。所有研究没有不良反应。结论:针灸治疗SNHL有突出的临床疗效,可明显提高有效率,并改善电测听听力。纳入文献质量偏低,在未来仍需更多高质量RCT试验验证结论。
Objective:To systematically evaluate the therapeutic effect of acupuncture on sensorineural hearing loss.Method:A randomized controlled or semi randomized controlled clinical study on acupuncture treatment of SNHL was conducted by searching five major databases:China National Knowledge Infrastructure(CNKI),Wanfang Data Academic Full Text Database(Wangfang),VIP Chinese Journal Full Text Database(VIP),SinoMed,and PubMed through a computer.Meta data analysis was conducted using Revman5.3 software to evaluate its clinical efficacy and improve the hearing effect of electrical audiometry.Result:After inclusion and exclusion criteria,a total of 23 articles were included,with a total of 2130 cases included.Compared with the treatment group(acupuncture)and the control group(Western medicine),the effective rates were 928/1121(82.8%)and 600/1009(59.4%),respectively.The meta-analysis results showed that acupuncture has a clear therapeutic effect on SNHL(OR=3.60,95%Cl(2.92,4.45),Z=11.92,P<0.0001),and the treatment group has a significant advantage in improving the electrical hearing loss value of SNHL(MD=-2.08,95%Cl(-3.40,-0.76),Z=11.92,P=0.002).No adverse events occurred in all studies.Conclusion:acupuncture and moxibustion has outstanding clinical efficacy in the treatment of SNHL,which can significantly improve the effective rate and improve the audiometric hearing.The quality of the included literature is relatively low,and more high-quality RCT experiments are still needed in the future to further validate the conclusions.
