摘要
目的探讨VEGF、s Flt-1、PLGF、MVD在早发型子痫前期患者胎盘中的表达与胎盘血管病变的关系,并与晚发型子痫前期做比较。方法随机选取2013年5月至2014年6月规律产检并分娩的子痫前期孕妇105例,其中早发型子痫前期46例,晚发型子痫前期59例。选取同期健康孕妇50例作为健康对照组,采用免疫组织化学SP法检测MVD、VEGF、PLGF、s Flt-1的表达,CD34双标免疫组化法标记血管检测胎盘中的微血管密度值,并进行相关性分析。结果早发型子痫前期患者胎盘组织中VEGF阳性表达率30.4%,晚发型组为61.0%,正常对照组为90%,3组差异有统计学意义(P<0.05);早发型子痫前期患者胎盘组织中s Flt-1阳性表达率80.4%,晚发型组76.3%,正常对照组14%,3组差异有统计学意义(P<0.05);早发型子痫前期患者胎盘组织中PLGF阳性表达率21.7%,晚发型组为64.0%,正常对照组88%,3组差异有统计学意义(P<0.05);早发型子痫前期患者胎盘组织中微血管密度(37.12±2.16),晚发型组(59.35±2.78),正常对照组(90.10±4.18),三组差异有统计学意义(P<0.05)。结论 s Flt-1在早发型子痫患者的胎盘组织中高表达,抑制了其靶组织VEGF及PLGF表达,影响了胎盘微血管形成,引发了子痫前期的发生,s Flt-1出现越早,子痫前期发生越早。
Objective:To explore the relationship between the genetic expression of VEGF,s Flt-1,PLGF in the placentas of patients with early onset preeclampsia and vessel diseases of placentas,compared with late onset ones. Methods:105 cases were randomly selected among all the patients with preeclampsia,who had come for regular antenatal care and delivery. Among those cases,46 were early onset while the rest 59 ones were late onset. Another 50 cases of healthy pregnant women were chosen as the control group. The immunohistochemistry SP method is utilized to detect the expression of MVD,VEGF,PLGF,and s Flt-1. Additionally,CD34 double indirect immunohistochemistry method is to detect the capillary density within the marked capillaries and furtherly the relativity analysis. Results:For the expression rate of VEGF in early onset preeclampsia cases,inside the placenta tissues,it is 30.4%;that in the late onset cases is 61.0%,and 90% for the control group. The difference is meaningful statistically(P0.05). For the expression rate of s Flt-1,it is 80.4% for early onset cases,76.3% for late onset cases,and 14% for control ones. The difference is statistically meaningful(P0.05). For the expression rate of PLGF,it is 21.7% for early onset cases,64.0% for late onset cases,and 88% for control ones. The difference is statistically meaningful(P0.05). Conclusion:s Flt-1is highly expressed in the early onset preeclampsia patients,which suppresses that of VEGF and PLGF inside its target tissue,affecting the formation of capillaries in placenta tissue and leading to the preeclampsia. The earlier the s Flt-1 appears,the sooner the preeclampsia occurs.
