微型外固定支架与掌骨板固定治疗Rolando骨折的三维有限元分析 被引量:6

摘要 目的比较微型外固定支架与微型掌骨板固定对Rolando骨折稳定性的影响。方法选择一名健康成年志愿者,对其右手及尺、桡骨远端进行三维CT扫描,并将扫描数据导入Mimics10.0软件中,对第一、二掌骨及大多角骨进行三维建模;所有数据以点云格式输入到Geomagic studio10软件中进行编辑,得到精细三维模型,以IGES格式保存。将其导入ANSYS12.0软件中,利用该软件的建模功能建造微型外固定支架及微型掌骨板模型,并进行Rolando骨折模拟。在遵循临床治疗的原则下分别用微型外固定支架与微型掌骨板对Rolando骨折进行固定,赋予各单位属性后建立三维有限元模型。在第一掌骨头沿纵轴施加载荷为100 N的节点压力,记录两种固定方式下掌尺侧骨折块、背外侧骨折块的相对位移和总位移。结果微型外固定支架固定掌尺侧骨折块相对位移为-0.242^-2.392 mm,总位移为0.638~5.986 mm;背外侧骨折块分别为-0.489^-2.935 mm、0.157~4.898 mm。微型掌骨板固定掌尺侧骨折块相对位移为0.152^-0.415 mm,总位移为0.187~0.648 mm;背外侧骨折块分别为0.186~0.314、0.158~0.554 mm。结论微型掌骨板对Rolando骨折的固定效果优于微型外固定支架。 Objective Using three-dimensional( 3D) finite element method to compare the effects of mini external fixation and mini metacarpal bone on the stability of Rolando fracture. Methods We chose a healthy adult volunteer and scanned CT of his right hand and distal radius. Then,we inputted the data into Mimics10. 0,and built the 3D model of the first and the second metacarpal and trapezium. After that,we inputted all the data into Geomagic studio10 for editing,and then transferred the models into ANSYS12. 0. The models of mini external fixation and mini metacarpal bone were established by using ANSYS12. 0 and underwent Rolando fracture simulation. 3D finite element models were established after using mini external fixation and mini metacarpal bone to fix. At the first metacarpal head,along the longitudinal axis,we applied 100 N nodes pressure,then recorded the pieces of palm and foot side fractures under these two kinds of fixation way,and the relative displacement and total displacement of dorsolateral fractures. Results With the miniature external fixation,the palm side fracture’s relative displacement was- 0. 242 to- 2. 392 mm,and the total displacement was 0. 638-5.986 mm. The dorsolateral displacement is- 0. 489 to- 2. 935 mm and 0. 157- 4. 898 mm,respectively. With the mini metacarpal bone plate,the palm side fracture’s relative displacement was 0. 152 to- 0. 415 mm,and the total displacement was 0. 187-0. 648 mm. The dorsolateral displacement was 0. 186- 0. 314 mm and 0. 158- 0. 554 mm,respectively.Conclusion The fixation effect of mini metacarpal bone plate is significantly better than that of the mini external fixation.
