应用微型钢板治疗三部分Rolando骨折 被引量:6

Treatment of three-part Rolando fracture with mini-plate

摘要 目的探讨应用微型钢板治疗三部分Rolando骨折的临床疗效。方法 2007年12月~2011年3月应用微型钢板治疗12例Rolando骨折,其中男性9例,女性3例,均行切开复位微型钢板螺钉内固定术。术后随访6~35个月,平均16个月。术后每月复诊至骨折愈合和功能基本恢复正常,记录指间活动、掌指和掌腕关节活动范围、握力、满意度、疼痛水平和关节炎改变,以手指总主动活动度法(TAM)和上肢功能调查量表(DASH)标准评价术后手指功能。结果 12例均获骨性愈合,平均愈合时间2.8个月,无切口感染、神经损伤、畸形愈合和骨关节炎表现。TAM功能评分优8例,良4例;优良率100%。DASH评分是3.4分。结论对掌骨基底部有2个大骨折块没有非常粉碎的三部分Rolando骨折应用微型钢板治疗,复位满意、固定可靠,功能恢复好,是一个较为理想的手术方式。 Objective To investigate the clinical efficacy of three - part Rolando fractures with mini-plates. Methods Totally 12 patients with three-part Rolando fractures treated by open reduction and internal fixation with mini-plates between Dec. 2007 and Mar. 2011 were retrospectively studied. All the patients, including 9 males and 3 females ,were followed up for average of 16 months ( range 6-35 months). The functions of metacarpophalangeal and interphalangeal joints were evaluated using TAM and DASH scales. Results Bony union was achieved in 12 patients. The average bony union time was 2.8 months. There were no complications such as wound infection, nerve injury, malunion or osteoarthritis. According to TAM scales, the excellent and good rate achieved 100 % and the DASH scale was 3.4. Conclusion Mini-plates approved to be a good option for the treatment of three-part Rolando fractures with two large fragments.
