摘要
目的分析参苓白术散加减治疗慢性阻塞性肺疾病稳定期肺脾气虚证的临床疗效。方法选取2014年2月~2015年9月在我院进行治疗的慢性阻塞性肺疾病稳定期肺脾气虚证118例患者,按照随机数字表法分为观察组和对照组各59例,对照组给予支气管舒张剂噻托溴铵粉雾剂吸入治疗,观察组在对照组的基础上加用中药参苓白术散加减治疗,观察两组患者的临床疗效、肺功能、呼吸肌耐力指标及营养状态前后变化。结果治疗前两组患者主症和次症中医证候积分比较无明显差异（P〉0.05）,治疗后均显著下降（P〈0.05）,且观察组下降幅度明显高于对照组（P〈0.05）;两组疗效等级比较差异有统计学意义（P〈0.05）,观察组总有效率94.92%明显高于对照组的71.19%（P〈0.05）;治疗前两组ALB和BMI、最大通气量MVV及6MWD比较,差异无统计学意义（P〉0.05）,治疗后均有明显升高（P〈0.05）,且观察组各项指标均明显优于对照组（P〈0.05）。结论参苓白术散加减治疗慢性阻塞性肺疾病稳定期肺脾气虚证效果显著,对于改善机体营养、增强运动耐力有重要作用。
Objective To analyze the clinical effects of shenling baizhu powder add and subtract in the treatment of lung spleen Qi deficiency syndrome of chronic obstructive pulmonary disease at stable stage.Methods 118 cases of patients with lung spleen Qi deficiency syndrome of chronic obstructive pulmonary disease at stable stage in our hospital from February 2014 to September 2015 were selected and divided into observation group and control group according to the random digital table method,with 59 cases in each group.The control group were treated with inhalation of ammonium bromide aerosol of bronchial diastolic agent,and the observation were treated with shenling baizhu powder add and subtract on the basis of the contreol group.The clinical curative effects and the changes of pulmonary function,respiratory muscle endurance indexes and nutritional status of the two groups were observed.Results There were no obvious differences in the scores of primary symptoms and secondary symptoms between the two groups before treatment（P〈0.05）,which were significantly decreased after treatment（P〈0.05）,and the descender of the observation group was significantly higher than that of the control group（P〈0.05）.The difference was statistically significant in the level comparision of clinical effects between the two groups（P〈0.05）,and the total effective rate of the observation group was94.92%,obviously higher than that of 71.19% of the control group（P〈0.05）.The differences were not statistically significant in the ALB and BMI,maximum ventilation rate MVV and 6MWD between the two groups before treatment（P〉0.05）,which were obviously increased after treatment（P〈0.05）,and the indexes in the observation group were obviously better than those in the control group（P〈0.05）.Conclusion Shenling baizhu powder add and subtract in the treatment of lung spleen Qi deficiency syndrome of chronic obstructive pulmonary disease at stable stage has remarkable effects,which plays an important role on reducing improving body nutritional status and raising sporting enduran.
