摘要
目的分析比较经皮撬拔内固定与切开复位内固定治疗跟骨骨折患者的疗效。方法将82例跟骨骨折患者随机分为两组,即观察组经皮撬拔内固定治疗,对照组经切开复位内固定治疗,再比较两组疗效。结果两组治疗优良率均为82.9%,差异无统计学意义(P>0.05);观察组住院时间为(6.02±2.12)d显著低于对照组,差异有统计学意义(P<0.05);观察组Maryland评分为(82.83±13.56)分与对照组比较,差异无统计学意义(P>0.05);两组患者术后1d、12个月Bohlers角、Gisanse角均显著高于术前,差异有统计学意义(P<0.05);两组患者术前、术后2d、12个月Bohlers角、Gisanse角比较,差异无统计学意义(P>0.05)。术后观察组并发症发生率为12.2%,与对照组比较,差异无统计学意义(P>0.05)。结论经皮撬拔内固定与切开复位治疗跟骨骨折,术后影像学指标相近,术后并发症少,疗效显著。但经皮撬拔内固定治疗,手术创伤较小,可利于足部功能恢复,值得临床推广应用。
Objective To analyze and compare the clinical effects of percutaneous poking internal fixation and open reduction and internal fixation in the treatment of patients with calcaneal fracture. Methods 82 patients with calcaneal fracture who were randomly divided into the two groups,the observation group were treated with percutaneous lever pulling and internal fixation while the control group were treated with open reduction and internal fixation. Then, the clinical effects of these two groups of patients were compared. Results The excellent and good rate of two groups were 82.9%,and there was no statistical difference（P〈0.05）;the hospitalization time in the observation group was（6.02±2.12）d which was significantly lower than that in the control group,and the differences had the statistical significance（P〈0.05）;the Maryland score in observation group of（82.83±13.56）score were compared with control group,and there was no statistical difference（P〈0.05）;In addition,after operation of 1D,12 months the Bohlers angle,Gisanse angle of two groups of patients were significantly higher than those before operation,and the differences had the statistical significance（P〈0.05）.Before surgery, postoperative of 2d,12 months,the Bohlers angle,Gisanse angle of two groups of patients were compared,there was no statistical significance（P〈0.05）.The incidence of postoperative complications of 12.2% which was compared with that of the control group,with no statistical significance（P〈0.05）. Conclusion The percutaneous internal fixation and open reduction in the treatment of calcaneal fractures,which the postoperative imaging parameters are similar,postoperative complications is less,and the clinical effect is significant. However, the treatment of percutaneous internal fixation and internal fixation is less traumatic,which is beneficial to the recovery of the function of the foot, and it is worthy of clinical application.
