红细胞分布宽度对评估重度颅脑损伤预后的意义 被引量:4

摘要 目的探讨红细胞分布宽度(RDW)与重度颅脑损伤患者预后的相关性。方法选取2010年3月至2014年12月本院收治的95例重度颅脑损伤患者为研究对象,按照24小时内RDW水平将患者分为RDW增高组(RDW>15.0%,n=22例)及非RDW增高组(RDW≤15.0%,n=73例);按照入院后第30天是否死亡分为存活组及死亡组;比较各组相关指标的差异。结果与RDW≤15.0%组比较,RDW>15.0%组患者入院时间较长、收缩压(SBP)及舒张压(SDP)偏低、入院GCS评分较低、APACHEⅡ评分较高、死亡率较高(P<0.05);与存活组比较,死亡组入院24小时内RDW水平较高,入院GCS评分较低,APACHEⅡ评分较高(P<0.05);患者30天死亡的多因素Logistic回归分析显示:RDW升高是重度颅脑损伤患者死亡的独立危险因素(P<0.05)。结论 RDW水平升高与重度颅脑损伤患者近期预后密切相关,可能是患者近期死亡的独立危险因素,早期检测RDW水平对于评估患者病情及近期预后具有一定意义。
