高湿环境胃蛋白酶原与幽门螺杆菌IgG抗体变化相关性研究

摘要 目的调查高湿地区世居人群血清中幽门螺杆菌(Hp)抗Hp免疫球蛋白G(IgG)抗体情况及其胃蛋白酶原水平的变化情况,了解高湿地区胃蛋白酶原与Hp感染的相关性。方法对136例血清标本进行胃蛋白酶原和Hp检测,胃蛋白酶原检测采用时间分辨荧光免疫法,Hp检测采用胶体金法。结果 Hp IgG抗体阳性组血清胃蛋白酶原Ⅰ水平为(143.7±87.57)μg/L,高于阴性组(123.89±72.47)μg/L,两组差异无统计学意义(P>0.05);Hp IgG抗体阳性组血清胃蛋白酶原Ⅱ水平为(19.48±13.88)μg/L,高于阴性组(12.55±9.78)μg/L,两组差异有统计学意义(P<0.01);Hp IgG抗体阳性组血清胃蛋白酶原Ⅰ/胃蛋白酶原Ⅱ为9.09±4.59,低于阴性组的11.48±4.76,两组差异有统计学意义(P<0.05);胃蛋白酶原和Hp IgG抗体检测对于世居高湿地区居民血清阳性率差异无统计学意义(P>0.05)。结论高湿Hp IgG抗体阳性人群血清胃蛋白酶原Ⅱ浓度较阴性人群明显增高,胃蛋白酶原Ⅰ/胃蛋白酶原Ⅱ较阴性人群明显降低,提示高湿环境居民胃底黏膜病变风险较大,应重视Hp和胃底黏膜的体检。 Objective To investigate the situation of serum Helicobacter pylori(Hp)IgG antibody and pepsinogen(PG)level change among native population in the high humidity area,and to understand the correlation between PG with Hp infection.Methods Serum level of pepsinogen was measured with time-resorved fluoroimmunoassay,and Helicobacter pylori antibody with immunogold filtration assay in 136 cases of serum samples.Results Serum PGⅠ level in the Hp IgG antibody positive group was(143.7±87.57)μg/L,which was higher than(123.89±72.47)μg/L in the Hp IgG antibody negative group,but without statistically significant difference(P〉0.05);the serum PGⅡ level in the Hp IgG antibody positive groupwas(19.48±13.88)μg/L,which was significantly higher than(12.55±9.78)μg/L in the Hp IgG antibody negative group,the difference was statistically significant(P〈0.01);the serum PGⅠ/PGⅡ level in the Hp IgG antibody positive group was 9.09±4.59,which was significantly lower than 11.48±4.76 in the Hp IgG antibody negative group,the difference was statistically significant(P〈0.05);The difference in test of Hp antibody and PG in serum had no statistically significant difference among native population in the high humidity area(P〉0.05).Conclusion The serum PGⅡ level in the Hp IgG antibody positive population is higher than that in the Hp IgG antibody negative population,while serum PGⅠ/PGⅡ is significantly decreased,which suggests that the risk of gastric fundus mucosal lesions among residents in high humidity environment is higher and the detection of Hp and gastric fundus mucosa should be paid more attention to.
