妊娠中期血清脂肪因子脂肪细胞型脂肪酸结合蛋白水平与妊娠期糖尿病的相关性 被引量:11

摘要 妊娠期糖尿病(GDM)是妊娠期间首次发现的以葡萄糖耐受不良为特征的一种常见代谢性疾病,GDM患者产后患2型糖尿病(T2DM)风险明显增高。在病因学上,GDM与T2DM有相似之处[1]。T2DM已被普遍接受为糖脂病[2]。肥胖导致脂肪组织增生、分布异常,游离脂肪酸(FFAs)水平升高,可抑制胰岛素信号传递,引起并加重胰岛素抵抗(IR),导致葡萄糖刺激的胰岛素分泌功能受损,促进胰岛β细胞凋亡。另外,脂肪组织作为内分泌组织,在肥胖状态下脂肪因子分泌失调。肥胖相关的“脂毒性”也参与了GDM的发病机制,并日益受到关注[3-4]。脂肪细胞型脂肪酸结合蛋白(AFABP,亦称为FABP4或aP2)是脂肪酸结合蛋白超家族的成员,最初认为只是一种细胞内蛋白,近年研究发现, AFABP作为脂质伴侣,参与脂肪酸转运,并调节激素敏感性脂肪酶的活性,是一种具有激素作用的新脂肪因子[5],并参与调节胰岛素敏感性[6-7],可独立预测T2DM及T2DM合并大血管并发症的发生[7-9],但AFABP在GDM患者中的研究却较少。本研究旨在探讨孕中期妇女血清AFABP及其他脂肪因子与GDM之间的关系,为GDM的发病机制和治疗提供新的思路。
