摘要
目的分析N末端脑钠肽前体(NT-pro BNP)水平及心肌肌钙蛋白I(c Tn I)、肌红蛋白(Mb)、肌酸激酶同工酶(CK-MB)、血清同型半胱氨酸(Hcy)、C反应蛋白(CRP)对老年慢性心力衰竭的诊断价值。方法选择2010年5月至2012年5月开封市龙海医院明确诊断为慢性心力衰竭的老年患者156例,同时选择50例老年健康体检者作为对照组,对所有受试者NT-pro BNP、c Tn I、Mb、CK-MB、Hcy、CRP指标进行检测。结果与对照组相比,不同心功能分级下,NT-pro BNP、c Tn I、Mb、CK-MB、Hcy和CRP均明显升高,差异均有统计学意义(均P<0.05)。NT-pro BNP、c Tn I、Mb、CK-MB、Hcy和CRP随着心功能分级的升高呈上升趋势。Pearson相关分析提示,NT-pro BNP与c Tn I、Mb、CKMB、Hcy、CRP均相关。结论 NT-pro BNP前体水平与c Tn I、Mb、CK-MB、Hcy、CRP指标对老年慢性心力衰竭有一定的诊断价值。
Objective To analyze the effects of N-terminal pro-brain natriuretic peptide( NT-pro BNP)level and cardiac troponin I( c Tn I),myoglobin( Mb),creatine kinase isoenzyme MB( CK-MB),serum homocysteine( Hcy),C-reactive protein( CRP) on chronic heart failure in elderly patients. Methods A total of 156 elderly patients diagnosed as chronic heart failure in Kaifeng Longhai Hospital from May 2010 to May 2012 were selected as the observation group,while another 50 healthy elderly people underwent physical examination during the same period in the hospital were selected as the control group. Levels of NT-pro BNP and c Tn I,Mb,CK-MB,Hcy,CRP of the two groups were detected and compared. Results Compared with the control group,levels of NT-pro BNP and c Tn I,Mb,CK-MB,Hcy,CRP of the patients in the observation group with different cardiac function grading were all significantly increased,the differences were statistically significant( P 0. 05). Levels of NT-pro BNP and c Tn I,Mb,CK-MB,Hcy,CRP increased along with the rise of cardiac function grading. Pearson correlation coefficient indicated levels of NT-pro BNP and c Tn I,Mb,CK-MB,Hcy,CRP were all correlated. Conclusion Levels of NT-pro BNP and c Tn I,Mb,CK-MB,Hcy,CRP have different impact on chronic heart failure in elderly patients and have diagnostic value. The combined detection of those indicators can guide the diagnosis and treatment of heart failure,therefore,it is suitable for clinical application.
