摘要
目的了解方庄社区卫生服务中心分时预约就诊模式的实施效果。方法分别选取2012年4月—2013年4月、2013年5月—2014年5月方庄社区卫生服务中心全科候诊区分时预约就诊实施前后患者各1 500例,对其实施前后满意度、挂号和候诊时间进行对比分析。结果实施前后分别回收有效问卷1 250、1 276份,有效回收率为83.33%、85.07%。实施前后患者对诊疗环境、诊疗时间、医患沟通充分、病情熟悉程度、候诊时间的满意度评分比较差异均有统计学意义(P<0.05)。综合满意率由实施前的89.12%(1 114/1 250)上升为实施后的97.26%(1 241/1 276),差异有统计学意义(χ2=12.18,P<0.01)。实施后患者挂号、候诊时间较实施前均缩短〔(7.3±1.6)min比(26.0±1.8)min、(8.7±2.1)min比(29.9±4.4)min〕,差异有统计学意义(P<0.05)。结论分时预约就诊模式对优化门诊就诊流程、缩短患者非医疗等候时间、提高患者满意度发挥重要作用。分时就诊、有序就医的门诊建设思路得以初步体现。
Objective To explore the practice effect of timeshare appointment treatment model in Fangzhuang community health service center. Methods Prior to the implementation of timeshare appointment treatment model, 1500 patients in general practice waiting area of Fangzhuang community health service center from April 2012 to April 2013 were selected as study subjects; after the implementation of timeshare appointment treatment model, another 1 500 patients in general practice waiting area of Fangzhuang community health service center from May 2013 to May 2014 were also selected as study subjects. The satisfaction degree of patients, waiting time for registration and waiting time for doctors were compared between two groups. Results Prior to the implementation of timeshare appointment treatment model, there were 1 250 effective questionnaires with a valid return rate of 83.33% ; after the implementation of timeshare appointment treatment model, there were 1 276 effective questionnaires with a valid return rate of 85.07%. There were significant differences in patients' satisfaction score for environment of diagnosis and treatment, time of diagnosis and treatment, doctor- patient communication, doctors' familiarity with illness and waiting time for doctors between two groups (P 〈 0. 05 ) . Comprehensive satisfaction rate before the implementation of timeshare appointment treatment model [ 89. 12% ( 1 114/1 250 ) ] was significantly lower than that [ 97. 26% ( 1 241/1 276) ] after the implementation of timeshare appointment treatment model (X2 = 12.18, P 〈 0.01 ) . The waiting time for registration and waiting time for doctors after the implementation of timeshare appointment treatment model were significantly shorter than those before the implementation of timeshare appointment treatment model, respectively [ (7. 3 ±1.6) minvs. (26.0±1.8) min, (8.7 ±2.1) min vs. (29.9 ±4.4) min, both P〈0.05] .Conclusion The timeshare appointment treatment model plays an important role on optimizing the outpatient service process, shortening the waiting time of patients, and improving patients' satisfaction. The idea of the constructing outpatient about timeshare appointment treatment and orderly treatment has achieved initial results.
