宫颈鳞状上皮病变中人乳头瘤病毒L1蛋白与P16、Ki-67的表达及意义 被引量:15

摘要 目的:研究宫颈鳞状上皮病变中人乳头瘤病毒(HPV)L1蛋白与P16、Ki-67的表达,探讨HPV L1蛋白与P16、Ki-67在宫颈病变发展过程中的意义。方法:采用免疫组织化学(IHC)方法检测宫颈鳞状上皮病变230例〔其中尖锐湿疣50例,宫颈上皮内瘤变(CIN)Ⅰ级60例,CINⅡ-Ⅲ级95例,浸润性鳞状细胞癌25例〕中HPV L1蛋白与P16、Ki-67的表达情况,并研究其相关性。结果:宫颈鳞状上皮病变中HPV L1的阳性率为43.91%。其中在尖锐湿疣中HPV L1的阳性率为62.00%,在CINⅠ中HPV L1的阳性率为70.00%,在CINⅡ-Ⅲ中HPV L1的阳性率为29.47%,浸润性鳞状细胞癌中无表达;宫颈鳞状上皮病变中P16的阳性率为76.52%,在尖锐湿疣中P16的阳性率为30.00%,在CINⅠ中P16的阳性率为70.00%,在CINⅡ-Ⅲ中P16的阳性率为98.95%,浸润性鳞状细胞癌中P16的阳性率为100.00%;宫颈鳞状上皮病变中Ki-67的阳性率为76.08%,在尖锐湿疣中Ki-67的阳性率为28.00%,在CINⅠ中Ki-67的阳性率为68.33%,在CINⅡ-Ⅲ中Ki-67的阳性率为100.00%,浸润性鳞状细胞癌中Ki-67的阳性率为100.00%。HPV L1的阳性率随宫颈病变的发展而呈下降趋势,P16、Ki-67的阳性率则呈现升高趋势。结论:HPV L1与宫颈鳞状上皮内瘤变密切相关;抑癌基因P16蛋白的表达上调参与宫颈鳞状细胞癌的发生、发展过程;联合检测HPV L1、P16、Ki-67对宫颈上皮内瘤变的诊断及分级具有重要意义。
