摘要
目的评价改良的腹腔镜下腹膜外途径前列腺癌根治术的手术技巧及临床效果。方法对2009年10月-2013年7月施行的106例腹腔镜下腹膜外途径前列腺癌根治术临床资料进行回顾性分析。前期31例按标准术式完成手术(标准组),后期75例在常规根治术基础上,进行技术改良(改良组)。比较两组的手术时间、出血量、控尿率和并发症发生率等临床指标。结果改良组手术时间、出血量和手术并发症发生率均比标准组少,而30 d、90 d及180 d控尿率均高于标准组。结论改良的腹腔镜下腹膜外途径前列腺癌根治术是一种安全有效的手术方法,术中的精细处理提高了手术效率,减少了术后并发症,并明显提高了患者术后控尿能力。
[ Objective ] To evaluate the technique and clinical outcomes of modified extransperitoneal laparo- scopic radical prostatectomy. [Methods] From October 2009 to July 2013, 106 cases of prostate cancer were ana- lyzed retrospectively, which were treated by extransperitoneal laparoscopic radical prostatectomy. At the beginning, 31 patients underwent the operation by the standard operation, while the other underwent technology improvement. The data such as operative time, amount of intraoperative bleeding, urinary continence rate and incidence of compli- cations were analyzed. [ Results ] The improved group had less operative time, amount of intraoperative bleeding, in- cidence of complications than the standard group. While the 30 d-urinary continence rate, 90 d-urinary continence rate, 180 d-urinary continence rate were higher in the improved group than in the standard group. [ Conclusion ] Modified extransperitoneal laparoscopic radical prostatectomy is efficient and safe approach for localized prostate cancer. The precise operation can reduce the complication and improve urinary continence function.
