摘要
就郊区农村地区的糖尿病管理模式进行探索,提出由三级、二级医院与社区医疗服务中心(社区卫生服务站点)联动的全程糖尿病管理模式,总结取得的成效和分析存在的问题,为中国糖尿病防治管理模式在上海城乡地区实施提供适用性经验。
To explore the diabetic management mode of the suburbs and to provide a whole diabetic management mode with multi-levels linkage of tertiary hospitals,secondary hospitals and community health service centers （community health stations）. Achievements were concluded and problems were analyzed in order to provide the appropriate experience for the Chinese diabetic management mode of prevention and treatment in Shanghai urban and rural areas.
