摘要
实施基本药物制度,在规范诊疗用药、节约医药资源方面发挥了积极作用。但由于基本药物的覆盖面有限、药品的生产配送脱节以及疾病治疗的个性化需求等因素影响,100%使用基本药物的规定可能使基层社区卫生服务机构的诊疗用药受到一定程度限制,不能完全满足诊疗需要。部分安全、有效、廉价的常见病多发病诊疗用药不在基本药物目录内时,一些患者在社区卫生服务机构得不到完善的治疗,将转往上一级医院就诊,逐渐导致基层医疗机构丢失患者,造成大医院虹吸现象。鉴于以上情况,国家在坚持基本药物制度的主体地位基础上,允许基层社区卫生服务机构配备一定比例的非基本药物,有效解决了基层社区卫生服务机构用药的供需矛盾,对社区卫生服务机构的基本医疗产生了积极影响。
The implementation of the essential drug system has played a positive role in standardizing diagnosis,treatment and medication,and saving medical resources.However,due to the limited coverage of essential drugs,disjointed production and distribution of drugs,personalized needs for disease treatment and other factors,the requirement of 100%use of essential drugs may restrict the diagnosis and treatment of grassroots community health service institutions to a certain extent and cannot fully meet the needs of diagnosis and treatment.When some safe,effective and cheap medicines for diagnosis and treatment of common diseases and frequently-occurring diseases are not included in the list of essential medicines,some patients cannot get perfect treatment in the community health service institutions and will be transferred to the first-level hospitals for treatment,which gradually leads to the loss of patients in the primary medical institutions and the siphon phenomenon in the large hospitals.Based on the situation above,on the basis of adhering to the dominant position of the essential medicine system,the state allows community-level health service institutions to be equipped with a certain proportion of non-basic drugs,which effectively solves the contradiction between supply and demand of drugs in community-level health service institutions and has a positive impact on the basic medical treatment in community-level health service institutions.
