摘要
目的探讨高血压脑出血(HICH)患者TNF-α表达与血肿周围组织细胞凋亡的关系。方法按发病到入院后手术时间,HICH患者40例分为A1组(<6h,8例)、A2组(6h～,10例)、A3组(12h～,8例)、A4组(24h～,8例)和A5组(≥48h,6例);以ELISA和免疫组化法检测血清和血肿周围组织TNF-α含量;以TUNEL法检测血肿周围组织凋亡细胞。健康体检者20例为对照组。结果 A1-A5组血清TNF-α含量均高于对照组(P<0.05);A3-A5组脑组织中TNF-α阳性细胞数多于A1组(P<0.05);A2-A5组血肿周围组织中细胞凋亡数量多于A1组(P<0.05)。线性相关性分析显示,HICH患者血肿周围组织TNF-α阳性表达细胞数与血清TNF-α含量、血肿周围组织细胞凋亡数量呈正相关(r=0.892,r=0.867,P<0.05)。结论 HICH患者血肿周围组织存在TNF-α高表达和血清TNF-α含量明显升高,血肿周围组织细胞凋亡明显,提示TNF-α可能与细胞凋亡有关。
Objective To investigate the relationship of tumor necrosis factor alpha（TNF-α） expression and cell apoptosis of hematoma-adjacent tissues in the patients with hypertensive intracerebral hemorrhage（HICH）. Methods On the basis of the time from onset to surgery,40 HICH patients were assigned into 5 groups of A1 （〈6 h, 8 cases）, A2 （6-11 h, 10 cases）, A3 （12-23 h, 8 cases） ,A4（24-47 h,8 cases） and A5（≥48 h,6 cases）. Serum expression of TNF-α was detected by double antibody clamp enzyme-linked immunosorbent assay, the cells positively expressing TNF-α in hematoma-adjacent tissues were counted by immunohistochemical method, and cell apoptosis of hematoma-adjacent tissues was examined by TUNEL method. Twenty healthy people（group B） were taken as the controls. The correlation of TNF-α expression and cell apoptosis of hematoma-adjacent tissues was analyzed. Results Serum expression of TNF-α was significantly higher in groups of A1 to A5 than that in group B（P〈0. 05）. The ceils positively expressing TNF-α in hematoma-adjacent tissues were more in groups of A3 to A5 than those in group AI（P〈0. 05）. The apoptosis ceils of hematoma-adjacent tissues were more in groups of A2 to A5 than those in group A1 （P〈0. 05）. Serum level of TNF-α expression was positively correlated to the ceils positively expressing TNF-α and apoptosis in hematoma-adjacent tissues（r=0. 892,r=0. 867,P〈0. 05）. Conclusion TNF-α is highly expressed in serum and hematoma-adjacent tissues in the patients with HICH, which indicates that TNF-α may be related to cell apoptosis.
