摘要
目的探讨在Gas Man软件的指导下,小儿腹腔镜手术中七氟烷吸入性麻醉的临床应用效果。方法选取2016至2017年在湖州市第一人民医院儿科住院的拟行腹腔镜手术的3~6岁患儿60例,将其随机分为对照组和七氟烷组(Sev组),每组儿童30人。对照组行常规麻醉方法进行手术,Sev组采用七氟烷进行麻醉。手术结束后评估两组患儿在入室(T1)、置入喉罩之前(T2)、置入喉罩之后(T3)、手术开始时(T4)、拔除喉罩(T5)时间点的心率(HR)、平均动脉压(MAP)、脉搏氧饱和度(SpO2)、脑电双频谱指数(BIS)的数值;评估两组患儿的意识消失时间、置入喉罩时间、拔除喉罩时间、苏醒时间及麻醉之后手术室停留时间(PACU),并进行统计分析。结果两组患儿在T1、T2、T3、T4、T5时间点的HR、MAP、BIS及SpO2间差异均无统计学意义(均P>0.05)。两组患儿的意识消失时间差异有统计学意义(t=24.361,P<0.05),而两组儿童的置入喉罩、拔除喉罩、苏醒及PACU时间比较差异均无统计学意义(均P>0.05)。结论在Gas Man软件指导下,行小儿腹腔镜手术过程中给予七氟烷吸入性麻醉的临床效果仅能够缩短其意识消失时间,在其他方面与未用Gas Man软件指导的麻醉方式无显著性改善。
Objective To explore the clinical effect of sevoflurane inhalation anesthesia under the guidance of Gas Man software in pediatric laparoscopic surgery.Methods We selected 60 children aged 3-6 years who were scheduled to undergo laparoscopic surgery in Huzhou First People’s Hospital from 2016 to 2017.They were randomly divided into control group and sevoflurane group(Sev group),with 30 children in each group.The control group underwent conventional anesthesia,while Sev group accepted sevoflurane in anesthesia.The heart rate(HR),mean arterial pressure(MAP),pulse oxygen saturation(SpO2)and bispectral index(BIS)of two groups were evaluated at the time point of entering operation room(T1),before inserting laryngeal mask(T2),after inserting laryngeal mask(T3),at the beginning of operation(T4),and after removing laryngeal mask(T5).The time of consciousness disappearance,laryngeal mask placement,laryngeal mask removal,recovery time and post-anesthesia care unit(PACU)stay time were analyzed statistically.Results There were no significant differences in HR,MAP,BIS,and SpO2 values between two groups(all P >0.05).There was significant difference in the time of disappearance of consciousness between two groups of children(t=24.361,P<0.05),but the differences in laryngeal mask placement,laryngeal mask removal,recovery time and PACU stay time were not significant(all P >0.05).Conclusion Under the guidance of Gas Man software,the clinical effect of sevoflurane inhalation anesthesia in pediatric laparoscopic surgery can only shorten the time of consciousness disappearance,and there is no significant improvement in other aspects compared with the anesthesia without the guidance of Gas Man software.
